Description
Overview of the medicine
Labegest 200 mg Tablet contains Labetalol, which is a combined alpha- and beta-adrenoceptor blocking agent used to treat hypertension and angina pectoris. It helps to lower blood pressure and reduce the heart's workload.
Uses & Indications
Dosage
Adults
Initial dose 100 mg twice daily, increasing to 200 mg twice daily after 2-3 days. Maintenance dose 200-400 mg twice daily, up to a maximum of 2.4 g daily in divided doses.
Elderly
Lower initial doses and slower titration may be appropriate, e.g., 50 mg twice daily.
Renal_impairment
No specific dosage adjustment required, but caution is advised in severe renal impairment.
How to Take
Take orally, with or without food. Consistent timing relative to meals is recommended for optimal absorption.
Mechanism of Action
Labetalol acts as a non-selective beta-adrenergic and selective alpha-1 adrenergic receptor blocker. It reduces peripheral vascular resistance by blocking alpha-1 receptors, and decreases heart rate and cardiac output by blocking beta-receptors. The combined effect leads to a reduction in blood pressure without reflex tachycardia.
Pharmacokinetics
Onset
Oral: 20-120 minutes
Excretion
Mainly excreted in urine as metabolites and, to a lesser extent, as unchanged drug. Some excretion via bile.
Half life
Approximately 6-8 hours
Absorption
Well absorbed from the GI tract (approximately 100%), but undergoes significant first-pass metabolism, resulting in approximately 25% bioavailability.
Metabolism
Extensively metabolized in the liver, primarily via conjugation with glucuronic acid.
Side Effects
Contraindications
- Bronchial asthma or history of obstructive airway disease
- Heart block greater than first degree (unless paced)
- Cardiogenic shock
- Severe bradycardia
- Decompensated heart failure
- Hypotension
Drug Interactions
Cimetidine
Increases Labetalol bioavailability.
Nitroglycerin
Increased hypotensive effect.
Beta-2 agonists
May reduce bronchodilator effect.
Tricyclic antidepressants
Increased risk of tremor.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms include severe hypotension, bradycardia, bronchospasm, and acute heart failure. Treatment involves symptomatic and supportive measures, including atropine for bradycardia, vasopressors for hypotension, and bronchodilators for bronchospasm. Hemodialysis is largely ineffective.
Pregnancy & Lactation
Pregnancy Category C/D (depending on trimester). Considered relatively safe for managing hypertension in pregnancy, especially pre-eclampsia. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor infant for bradycardia or lethargy.
