Description
Overview of the medicine
Labetalol injection is an alpha- and beta-adrenergic blocking agent indicated for the control of blood pressure in severe hypertension, including hypertensive emergencies. It works by relaxing blood vessels and slowing heart rate.
Uses & Indications
Dosage
Adults
Initial: 20 mg IV bolus over 2 minutes. May be repeated every 10 minutes or continuous infusion 0.5-2 mg/min. Max: 300 mg total.
Elderly
Start with lower doses, closely monitor blood pressure and heart rate.
Renal_impairment
No dose adjustment typically required, but monitor closely.
How to Take
Administered intravenously, either as a slow bolus injection or a continuous infusion, usually in a hospital setting under close monitoring.
Mechanism of Action
Labetalol acts as a non-selective beta-adrenergic receptor blocker and a selective alpha-1 adrenergic receptor blocker. This dual action leads to vasodilation (via alpha-1 blockade) and reduced heart rate and cardiac output (via beta blockade), resulting in decreased blood pressure.
Pharmacokinetics
Onset
Within 5 minutes (IV bolus).
Excretion
Mainly renal excretion of metabolites and unchanged drug.
Half life
Approximately 5-8 hours.
Absorption
Rapid and complete absorption after intravenous administration.
Metabolism
Extensive hepatic metabolism, primarily by conjugation with glucuronic acid.
Side Effects
Contraindications
- Bronchial asthma or history of obstructive airway disease
- Overt cardiac failure
- Cardiogenic shock
- Second- or third-degree heart block
- Severe bradycardia
- Hypersensitivity to labetalol
Drug Interactions
Cimetidine
Increases labetalol bioavailability.
Nitroglycerin
Exaggerated hypotensive effect.
Tricyclic Antidepressants
May increase risk of tremor.
Calcium Channel Blockers (e.g., Verapamil, Diltiazem)
Increased risk of bradycardia and AV block.
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
Symptoms include severe hypotension, bradycardia, cardiac failure, and bronchospasm. Management involves supportive measures, IV fluids, atropine for bradycardia, glucagon, and vasopressors if necessary.
Pregnancy & Lactation
Pregnancy Category C. Used for severe hypertension in pregnancy; however, caution is advised. Excreted in breast milk, use with caution during lactation.
