Description
Overview of the medicine
Levomax 0.5% Eye Drop is a fluoroquinolone antibiotic used to treat bacterial infections of the eye, such as bacterial conjunctivitis and keratitis.
Uses & Indications
Dosage
Adults
For bacterial conjunctivitis: Instill 1-2 drops into the affected eye(s) every 2 hours while awake, up to 8 times per day, for the first 2 days, then 1-2 drops every 4 hours while awake, up to 4 times per day, from day 3 through day 7. For bacterial keratitis: Instill 1 drop into the affected eye(s) every 30 minutes to 1 hour while awake for the first 1-2 days, then every 1-2 hours while awake for 3-5 days, then reduce frequency as directed by physician.
Elderly
No dosage adjustment is typically required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for topical ophthalmic use in patients with renal impairment due to minimal systemic absorption.
How to Take
Wash hands thoroughly before use. Tilt your head back, gently pull down your lower eyelid to form a pouch, and instill the prescribed number of drops into the pouch. Close your eye(s) gently for 1-2 minutes. Avoid touching the dropper tip to your eye or any other surface to prevent contamination.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination, leading to bacterial cell death.
Pharmacokinetics
Onset
Clinical improvement typically observed within 2-5 days of initiation of treatment.
Excretion
Mainly eliminated via tears and minimal systemic renal excretion for the small absorbed fraction.
Half life
Systemic half-life is approximately 6-8 hours, but for topical ophthalmic use, local half-life in ocular tissues is more relevant, with sustained therapeutic concentrations.
Absorption
Following topical ocular administration, levofloxacin is absorbed into the ocular tissues (cornea, aqueous humor, iris, ciliary body) with minimal systemic absorption. Peak concentrations in tears are rapidly achieved.
Metabolism
Minimal systemic metabolism. Primarily unmetabolized.
Side Effects
Contraindications
- •Hypersensitivity to levofloxacin, other quinolones, or any component of the formulation.
- •Viral and fungal infections of the eye (as antibiotics are not effective against them).
Drug Interactions
Other topical ocular medications
If using other topical ophthalmic medications, wait at least 5 minutes between applying each medication to allow for proper absorption and avoid washout.
Storage
Store at room temperature (below 30°C). Protect from light. Do not freeze. Keep out of reach of children.
Overdose
Topical overdose is unlikely to cause systemic toxicity due to limited absorption. If accidentally ingested, drink fluids and seek medical attention if symptoms occur.
Pregnancy & Lactation
Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Caution should be exercised when Levomax 0.5% Eye Drop is administered to a nursing mother, as levofloxacin is excreted in human milk after systemic administration. Consult a doctor.
