Overview of the medicine
Levomax 500 mg Tablet contains Levofloxacin, a fluoroquinolone antibiotic used to treat a variety of bacterial infections. It works by stopping the growth of bacteria. This medicine is prescribed for conditions like pneumonia, bronchitis, sinusitis, skin and urinary tract infections.
Uses & Indications
Dosage
Adults
Typically 500 mg once daily for 7-14 days depending on the infection type and severity. For some severe infections, 750 mg once daily may be prescribed.
Elderly
No specific dose adjustment needed based solely on age; however, renal function should be assessed and dose adjusted if impaired.
Renal_impairment
Dosage adjustment required. For CrCl 20-49 mL/min, initial dose 500 mg, then 250 mg every 24 hours. For CrCl <20 mL/min, initial dose 500 mg, then 250 mg every 48 hours.
How to Take
Orally, with or without food. Swallow the tablet whole with a full glass of water. Do not crush, chew, or split the tablet. Maintain adequate hydration.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, essential enzymes for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.
Pharmacokinetics
Onset
Varies by infection type, generally within hours to days.
Excretion
Primarily excreted unchanged in the urine, mainly via glomerular filtration and tubular secretion (about 87% of the dose within 48 hours).
Half life
Approximately 6-8 hours.
Absorption
Rapid and complete oral absorption. Peak plasma concentrations reached within 1-2 hours. Bioavailability is approximately 99%.
Metabolism
Minimally metabolized in the liver (less than 5% of a dose).
Side Effects
Contraindications
- •Hypersensitivity to levofloxacin, other fluoroquinolones, or any component of the formulation
- •History of tendinitis or tendon rupture associated with fluoroquinolone use
- •Epilepsy or other seizure disorders
- •Children and adolescents (under 18 years of age) due to risk of arthropathy
- •Pregnancy and breastfeeding
Drug Interactions
Warfarin
May enhance anticoagulant effects; monitor INR.
Corticosteroids
Increased risk of tendinitis and tendon rupture.
Insulin/Oral Hypoglycemics
May cause dysglycemia (hypoglycemia or hyperglycemia), monitor blood glucose.
NSAIDs (Nonsteroidal Anti-inflammatory Drugs)
May increase the risk of CNS stimulation and convulsions.
Antacids (magnesium/aluminum), Sucralfate, Iron/Zinc supplements
Decreased absorption of levofloxacin. Administer levofloxacin at least 2 hours before or 2 hours after these agents.
Storage
Store at room temperature (below 30°C), protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include CNS effects (confusion, tremors, seizures, hallucinations), QT prolongation, gastrointestinal disturbances (nausea, vomiting), and crystalluria. Treatment is symptomatic and supportive. Gastric lavage and adequate hydration may be helpful. Hemodialysis or peritoneal dialysis is not effective in removing levofloxacin.
Pregnancy & Lactation
Not recommended during pregnancy due to potential risks to the fetus, including musculoskeletal abnormalities. Avoid use during breastfeeding as levofloxacin is excreted in breast milk and may cause adverse effects in the infant.
