Overview of the medicine
Levomax 750 mg Tablet contains Levofloxacin, a synthetic broad-spectrum antibacterial agent of the fluoroquinolone class. It is used to treat various bacterial infections.
Uses & Indications
Dosage
Adults
750 mg once daily for 5-14 days depending on infection type and severity. For CAP, 750 mg once daily for 5 days. For cUTI/Pyelonephritis, 750 mg once daily for 5 days.
Elderly
No specific dose adjustment based on age alone, but renal function should be assessed.
Renal_impairment
CrCl 20-49 mL/min: Initial 750 mg, then 250 mg every 2 days. CrCl 10-19 mL/min: Initial 750 mg, then 250 mg every 2 days. Hemodialysis/CAPD: Initial 750 mg, then 250 mg every 2 days.
How to Take
Take orally, with or without food. Ensure adequate fluid intake. Do not crush or chew the tablet.
Mechanism of Action
Levofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, enzymes necessary for bacterial DNA replication, transcription, repair, and recombination. This leads to bacterial cell death.
Pharmacokinetics
Onset
Within hours of administration, clinical improvement typically within 2-3 days.
Excretion
Primarily renal, mainly through glomerular filtration and tubular secretion. Approximately 87% of an oral dose is recovered unchanged in urine within 48 hours.
Half life
Approximately 6-8 hours.
Absorption
Rapid and complete absorption after oral administration; peak plasma concentrations reached within 1-2 hours.
Metabolism
Minimally metabolized; primarily excreted unchanged.
Side Effects
Contraindications
- •Hypersensitivity to levofloxacin or other fluoroquinolones
- •History of tendinitis or tendon rupture associated with fluoroquinolone use
- •Epilepsy
- •Children and adolescents (under 18 years of age) - generally, due to risk of cartilage damage
Drug Interactions
NSAIDs
May increase the risk of CNS stimulation and convulsions.
Warfarin
Increased anticoagulant effects, monitor INR.
Corticosteroids
Increased risk of tendinitis/tendon rupture.
Antidiabetic agents (e.g., insulin, glyburide)
Risk of dysglycemia (hypoglycemia or hyperglycemia), monitor blood glucose.
Antacids containing magnesium or aluminum, sucralfate, metal cations (e.g., iron, zinc), didanosine (buffered tablets or pediatric powder for oral solution)
Reduced oral absorption of levofloxacin. Take levofloxacin at least 2 hours before or 2 hours after these agents.
Storage
Store at room temperature (below 30°C), protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include CNS effects (confusion, hallucinations, tremors, convulsions) and QT prolongation. Treatment is supportive and symptomatic; hemodialysis or peritoneal dialysis is not effective.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Levofloxacin is excreted in human milk; use caution if administering to a nursing mother.
