Overview of the medicine
Lipitin 20 mg Tablet contains Atorvastatin, a medication used to lower cholesterol and triglycerides in the blood. It helps prevent heart attacks, strokes, and other heart-related problems.
Uses & Indications
Dosage
Adults
Initial dose 10-20 mg once daily. Max 80 mg daily. For primary hypercholesterolemia, usual dose is 10 mg once daily.
Elderly
No dose adjustment required.
Renal_impairment
No dose adjustment required.
How to Take
Take orally once daily, with or without food, at any time of day, but preferably at the same time each day.
Mechanism of Action
Atorvastatin selectively inhibits HMG-CoA reductase, an enzyme that catalyzes the early rate-limiting step in cholesterol biosynthesis. This results in a compensatory increase in the expression of hepatic LDL receptors, which leads to increased clearance of LDL-C from the blood.

Onset
Lipid-lowering effect seen within 2 weeks, maximal effect in 4 weeks.
Excretion
Primarily excreted in bile following hepatic and/or extrahepatic metabolism.
Half life
Elimination half-life of Atorvastatin is approximately 14 hours; active metabolites contribute to ~20-30 hours of HMG-CoA reductase inhibitory activity.
Absorption
Rapidly absorbed after oral administration; absolute bioavailability is approximately 14%.
Metabolism
Extensively metabolized by cytochrome P450 3A4 (CYP3A4) to active ortho- and parahydroxylated metabolites and various beta-oxidation products.

Drug Interactions
Gemfibrozil, other Fibrates, Niacin
Increased risk of myopathy/rhabdomyolysis.
Cyclosporine, Telaprevir, Tipranavir/Ritonavir
Increased risk of myopathy/rhabdomyolysis due to increased Atorvastatin exposure. Avoid concomitant use.
Clarithromycin, Itraconazole, Protease Inhibitors (e.g., Ritonavir)
Increased Atorvastatin plasma concentrations. Dose adjustment of Atorvastatin may be necessary.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
No specific antidote. In case of overdose, provide symptomatic and supportive measures. Monitor liver function and CK levels.
Pregnancy & Lactation
Category X in pregnancy (contraindicated due to potential fetal harm). Contraindicated during lactation as Atorvastatin is excreted in human milk.
