Description
Overview of the medicine
Loratadine is a non-sedating, long-acting antihistamine used to relieve symptoms of seasonal allergic rhinitis, perennial allergic rhinitis, and chronic idiopathic urticaria in children.
Uses & Indications
Dosage
Adults
For children 2-5 years: 5 mg (5 mL of 1 mg/mL solution) once daily. For children 6 years and older: 10 mg (10 mL of 1 mg/mL solution) once daily. Consult a physician for exact dosing for Loridon 5 mg Pediatric Drop.
Elderly
Not applicable for pediatric drop. Refer to adult formulations for elderly dosage.
Renal_impairment
For patients with severe renal impairment (glomerular filtration rate < 30 mL/min), the initial dose should be reduced to 5 mg (5 mL) every other day. Consult physician.
How to Take
Administer orally once daily, with or without food. Use the provided oral syringe or calibrated dropper for accurate measurement of the prescribed dose. Do not use household teaspoons, as they may not provide accurate dosing.
Mechanism of Action
Loratadine selectively blocks peripheral H1-histamine receptors, thereby preventing the binding of histamine and reducing histamine-induced symptoms like sneezing, runny nose, itching, and watery eyes.
Pharmacokinetics
Onset
Onset of action typically occurs within 1-3 hours.
Excretion
Approximately 40% of the dose is excreted in the urine and 42% in the feces over 10 days, primarily as conjugated metabolites.
Half life
Loratadine has a plasma half-life of approximately 8-11 hours. Its active metabolite, desloratadine, has a longer half-life of 17-24 hours.
Absorption
Rapidly absorbed from the gastrointestinal tract following oral administration. Peak plasma concentrations are reached within 1-2 hours for loratadine and 1.5-3.7 hours for desloratadine (active metabolite).
Metabolism
Loratadine is extensively metabolized in the liver by cytochrome P450 3A4 and 2D6 into its major active metabolite, descarboethoxyloratadine (desloratadine).
Side Effects
Contraindications
- •Known hypersensitivity to loratadine, desloratadine, or any component of the formulation.
- •Children under 2 years of age without specific physician consultation.
Drug Interactions
Cimetidine
Use with cimetidine may slightly increase loratadine plasma levels; clinical significance is generally negligible.
Erythromycin
Co-administration with erythromycin may lead to increased plasma concentrations of loratadine and desloratadine, but without clinically significant changes in the ECG or adverse event profile.
Ketoconazole
Concomitant use with ketoconazole may increase plasma concentrations of loratadine and its active metabolite, desloratadine. However, no clinically significant adverse effects have been reported.
Storage
Store at controlled room temperature (20-25°C or 68-77°F). Protect from excessive moisture and direct light. Do not freeze. Keep the bottle tightly closed.
Overdose
Symptoms of overdose may include somnolence, tachycardia, and headache. In case of overdose, symptomatic and supportive treatment should be initiated immediately. Activated charcoal may be administered and gastric lavage should be considered if appropriate.
Pregnancy & Lactation
Pregnancy Category B. Animal reproduction studies have not demonstrated a risk to the fetus, but there are no adequate and well-controlled studies in pregnant women. This medicine should be used during pregnancy only if clearly needed. Loratadine and its active metabolite are excreted in breast milk; therefore, caution should be exercised when administered to a nursing woman.
