Description
Overview of the medicine
Macery 125 mg Suspension is an antibiotic from the macrolide class, used to treat a wide variety of bacterial infections, including respiratory tract infections, skin infections, ear infections, and sexually transmitted diseases.
Uses & Indications
Dosage
Adults
Dosage for children varies based on weight and infection type. General recommendation for most infections: 10 mg/kg body weight once daily for 3 days, or 10 mg/kg on day 1 followed by 5 mg/kg on days 2-5. (Adult dosing is typically higher for Azithromycin, consult specific adult formulations).
Elderly
No specific dose adjustment is required for elderly patients with normal renal and hepatic function. However, caution is advised due to potential for prolonged QT interval.
Renal_impairment
Mild to moderate renal impairment (creatinine clearance > 10 mL/min): No dose adjustment. Severe renal impairment (creatinine clearance < 10 mL/min): Use with caution, as data are limited.
How to Take
Macery suspension is for oral administration. It can be taken with or without food. Shake the bottle well before each use to ensure uniform suspension. Use the provided measuring device to ensure accurate dosing.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby interfering with the translocation of peptides.
Pharmacokinetics
Onset
Clinical effects typically begin within 2-3 days, with peak plasma concentrations occurring within 2-3 hours after administration.
Excretion
Primarily excreted unchanged in the bile; a small percentage is excreted in urine.
Half life
The terminal elimination half-life is long, approximately 2-4 days, which allows for once-daily dosing and shorter treatment durations.
Absorption
Rapidly absorbed from the gastrointestinal tract. Peak plasma concentrations are reached within 2-3 hours. Bioavailability is approximately 37%.
Metabolism
Primarily metabolized in the liver by N-demethylation to inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, or any macrolide or ketolide antibiotic.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
May increase serum digoxin levels; monitor digoxin levels.
Warfarin
Increased anticoagulant effects have been reported; monitor prothrombin time carefully.
Antacids (containing aluminum or magnesium)
May reduce peak plasma concentrations of azithromycin; administer azithromycin at least 1 hour before or 2 hours after antacids.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children. After reconstitution, store at room temperature (15-30°C) or in a refrigerator (2-8°C) as per specific product instructions, and discard after 5-10 days.
Overdose
Symptoms of overdose may include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures. Gastric lavage and general supportive measures are indicated as necessary.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. Excreted in breast milk; use with caution during lactation after weighing benefits against risks.
