Description
Overview of the medicine
Macery 250 mg Tablet contains Azithromycin, a macrolide antibiotic used to treat various bacterial infections including respiratory tract infections, skin and soft tissue infections, and sexually transmitted diseases. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For most infections, 500 mg once daily for 3 days, or 500 mg on day 1 followed by 250 mg once daily for 4 days. For certain STIs, a single 1 gm dose.
Elderly
No specific dosage adjustment is required for elderly patients.
Renal_impairment
No dosage adjustment is necessary for mild to moderate renal impairment. Caution is advised in severe renal impairment (CrCl < 10 mL/min).
How to Take
Macery 250 mg Tablet can be taken with or without food. It should be swallowed whole with water.
Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of susceptible microorganisms, thereby inhibiting protein synthesis. This results in the inhibition of bacterial growth and replication, leading to its bacteriostatic effects.
Pharmacokinetics
Onset
Within 2-3 hours, but full therapeutic effect may take longer.
Excretion
Mainly excreted unchanged in bile (up to 50%) and a small amount via urine (about 6%).
Half life
Terminal elimination half-life is 2-4 days (48-96 hours), due to extensive tissue uptake and slow release.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%. Peak plasma concentrations occur 2-3 hours after administration.
Metabolism
Primarily metabolized in the liver to inactive metabolites; CYP3A4 is involved to a minor extent.
Side Effects
Contraindications
- •Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or any component of the formulation.
- •History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
Increased serum digoxin levels have been reported; monitor digoxin levels.
Antacids
Concurrent administration with aluminum- and magnesium-containing antacids can reduce peak plasma levels of azithromycin. Take azithromycin at least 1 hour before or 2 hours after antacids.
Warfarin
Increased anticoagulant effects have been reported; monitor INR carefully.
Nelfinavir
Increased azithromycin concentrations; no dosage adjustment needed.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
In case of overdose, symptomatic and supportive measures are recommended. Gastric lavage and general supportive measures may be indicated. There is no specific antidote.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. It is excreted in human milk; use with caution in nursing mothers.
