Overview of the medicine
Macrozith 500 mg Tablet contains Azithromycin, a macrolide antibiotic used to treat a wide range of bacterial infections including respiratory tract infections, skin and soft tissue infections, and sexually transmitted diseases. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For respiratory tract, skin and soft tissue infections: 500 mg once daily for 3 days, or 500 mg on Day 1 followed by 250 mg once daily on Days 2-5. For Chlamydia trachomatis: 1000 mg as a single oral dose.
Elderly
No dosage adjustment needed for elderly patients with normal renal and hepatic function.
Renal_impairment
No dosage adjustment is necessary for patients with mild to moderate renal impairment (creatinine clearance >10-80 mL/min).
How to Take
Take Macrozith 500 mg Tablet orally, with or without food. To minimize gastric upset, it can be taken with food.
Mechanism of Action
Azithromycin inhibits bacterial protein synthesis by binding to the 50S ribosomal subunit of susceptible microorganisms, thereby interfering with the translation of mRNA.

Onset
Peak plasma concentrations reached in 2-3 hours.
Excretion
Mainly excreted unchanged in bile; a small amount is excreted via urine.
Half life
Prolonged terminal elimination half-life of 2-4 days.
Absorption
Rapidly absorbed from the gastrointestinal tract; bioavailability is approximately 37%.

Primarily hepatic, but limited metabolism to inactive metabolites.

Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any component of the formulation.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Digoxin
Increased serum digoxin levels, monitor digoxin concentrations.
Antacids
Reduced peak plasma concentrations of azithromycin; administer azithromycin at least 1 hour before or 2 hours after antacids.
Warfarin
Increased anticoagulant effect, monitor INR.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves gastric lavage and general supportive measures.

Pregnancy Category B. Studies in animals have shown no evidence of harm to the fetus. Use during pregnancy only if clearly needed. Azithromycin is excreted in human milk; use with caution in nursing mothers.
