Description
Overview of the medicine
Maczith 500 mg Tablet contains Azithromycin, a macrolide antibiotic used to treat a wide range of bacterial infections. It works by stopping the growth of bacteria.
Uses & Indications
Dosage
Adults
For most infections: 500 mg once daily for 3 days, or 500 mg on day 1 followed by 250 mg daily on days 2-5. For STIs: a single 1 g dose.
Elderly
No specific dose adjustment is required for elderly patients unless they have severe renal or hepatic impairment.
Renal_impairment
No dose adjustment is needed for mild to moderate renal impairment (creatinine clearance >40 mL/min). Use with caution in severe renal impairment.
How to Take
Take Maczith 500 mg tablet orally, with or without food, as a single daily dose. Swallow the tablet whole with water. Do not crush or chew.
Mechanism of Action
Azithromycin binds to the 50S ribosomal subunit of susceptible microorganisms, inhibiting protein synthesis and thus interfering with the bacteria's ability to multiply.
Pharmacokinetics
Onset
Onset of antibacterial activity typically within 2-3 hours, but clinical effects may take longer to manifest.
Excretion
Mainly excreted unchanged in the bile (about 50%), with a small amount (about 6%) excreted in the urine.
Half life
The terminal elimination half-life is approximately 2 to 4 days, allowing for once-daily dosing.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations occurring within 2-3 hours. Bioavailability is approximately 37%.
Metabolism
Primarily metabolized in the liver by N-demethylation, but less than 10% of the dose is metabolized.
Side Effects
Contraindications
- Hypersensitivity to azithromycin, erythromycin, any macrolide or ketolide antibiotic, or to any component of the formulation.
- History of cholestatic jaundice/hepatic dysfunction associated with prior azithromycin use.
Drug Interactions
Warfarin
Azithromycin may potentiate the anticoagulant effects of warfarin. Monitor INR closely.
Nelfinavir
Coadministration significantly increases azithromycin serum concentrations. Monitor for azithromycin side effects.
Antacids (containing aluminum or magnesium)
Can decrease the peak plasma concentrations of azithromycin. Take azithromycin at least 1 hour before or 2 hours after antacids.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include severe nausea, vomiting, diarrhea, and reversible hearing loss. Management involves symptomatic and supportive measures, including gastric lavage and close monitoring of vital signs.
Pregnancy & Lactation
Pregnancy Category B. Azithromycin should be used during pregnancy only if clearly needed. It is excreted in human breast milk; caution should be exercised when azithromycin is administered to a nursing mother.

Lab Monitoring
- Liver function tests (ALT, AST, bilirubin) in patients with pre-existing hepatic impairment or during prolonged treatment.
- Renal function tests (creatinine, BUN) in patients with severe renal impairment.
- ECG monitoring for QT interval in patients at risk of cardiac arrhythmias.
Doctor Notes
- Emphasize the importance of completing the full antibiotic course to prevent therapeutic failure and the development of antimicrobial resistance.
- Counsel patients regarding potential cardiac risks (QTc prolongation) and advise them to seek immediate medical attention if they experience palpitations, dizziness, or syncope.
- Be aware of potential drug interactions, especially with warfarin and antacids. Advise patients on appropriate spacing of doses.
Patient Guidelines
- Take the medicine exactly as prescribed by your doctor and complete the full course of treatment, even if you feel better.
- Do not share this medicine with others, even if they have similar symptoms.
- Report any severe side effects, especially signs of liver problems or severe allergic reactions, to your doctor immediately.
