Description
Overview of the medicine
Merocil (Meropenem) is a broad-spectrum carbapenem antibiotic used to treat a wide variety of serious bacterial infections, including pneumonia, complicated skin and intra-abdominal infections, and meningitis.
Uses & Indications
Dosage
Adults
Usually 500 mg to 1 gm IV every 8 hours, depending on the severity and type of infection. For meningitis or severe infections, 2 gm IV every 8 hours.
Elderly
Dose adjustment may be necessary based on renal function. Monitor renal function.
Renal_impairment
Dose adjustments required based on creatinine clearance: CrCl 26-50 mL/min, reduce dose by half; CrCl 10-25 mL/min, reduce dose to quarter; CrCl <10 mL/min, reduce dose to one-eighth.
How to Take
Administered by intravenous (IV) infusion over 15-30 minutes or by IV bolus injection (500 mg or 1 gm) over 3-5 minutes.
Mechanism of Action
Meropenem is a bactericidal antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Rapid, typically within minutes of IV administration.
Excretion
Primarily excreted unchanged in the urine (approximately 70% of the dose within 12 hours).
Half life
Approximately 1 hour in adults with normal renal function.
Absorption
100% bioavailable after intravenous administration.
Metabolism
Metabolized by hydrolysis of the beta-lactam ring to an inactive metabolite. Minimal hepatic metabolism.
Side Effects
Contraindications
- •Hypersensitivity to meropenem or other carbapenem antibiotics.
- •Severe hypersensitivity (e.g., anaphylactic reaction, severe skin reaction) to any other beta-lactam antibacterial agent (e.g., penicillins, cephalosporins).
Drug Interactions
Probenecid
Probenecid competes with meropenem for renal tubular secretion, resulting in increased plasma concentrations and prolonged half-life of meropenem.
Oral anticoagulants
Co-administration with meropenem may enhance the anticoagulant effect of warfarin and other oral anticoagulants.
Valproic acid/Sodium valproate
Meropenem can significantly decrease valproic acid levels, leading to loss of seizure control. Co-administration is generally not recommended.
Storage
Store below 30°C. Protect from light. Do not freeze. Reconstituted solutions should be used immediately or within specified stability periods.
Overdose
In case of overdose, meropenem can be removed by hemodialysis. Treatment should be supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Meropenem is excreted in small amounts into human milk; caution should be exercised when administered to a nursing woman.
