Merocil 250 mg Injection contains Meropenem, a broad-spectrum carbapenem antibiotic used to treat a wide variety of serious bacterial infections, including those resistant to other antibiotics. It is administered intravenously.
Uses & Indications
Dosage
Adults
For 250 mg strength, it is usually 250 mg IV every 8 hours for specific susceptible infections, or as directed by a physician. Doses may range from 500 mg to 1 g IV every 8 hours for more severe infections, based on infection type and severity.
Elderly
Dosage adjustment is recommended in elderly patients based on renal function (creatinine clearance).
Renal_impairment
Dose adjustment required based on creatinine clearance: e.g., CrCl 26-50 mL/min: 250 mg every 12 hours; CrCl 10-25 mL/min: 250 mg every 24 hours. Hemodialysis patients should receive dose after dialysis.
How to Take
Merocil 250 mg Injection is for intravenous use only. It should be administered by slow intravenous infusion over 15 to 30 minutes, or by intravenous bolus injection (5-20 mL) over 3 to 5 minutes.
Mechanism of Action
Meropenem inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death. It has a high stability to hydrolysis by most beta-lactamases.
Pharmacokinetics
Onset
Rapid, typically within minutes of IV administration.
Excretion
Primarily renal, with approximately 70% eliminated unchanged in urine within 12 hours. Hemodialysis removes meropenem.
Half life
Approximately 1 hour in adults with normal renal function.
Absorption
Rapidly absorbed after intravenous (IV) administration. Peak plasma concentration achieved at the end of infusion.
Metabolism
Minimally metabolized; about 20% by hydrolysis of the beta-lactam ring to an inactive metabolite.
Side Effects
Contraindications
- •Hypersensitivity to meropenem or any other carbapenem antibiotics.
- •Hypersensitivity to any other component of the formulation.
- •Patients with a history of severe hypersensitivity reaction to any other beta-lactam antibacterial agent (e.g., penicillins, cephalosporins).
Drug Interactions
Probenecid
Increases meropenem plasma concentrations and prolongs its half-life by inhibiting renal tubular secretion.
Valproic Acid / Sodium Valproate
Meropenem may decrease serum concentrations of valproic acid, potentially leading to loss of seizure control. Co-administration is generally not recommended.
Oral Anticoagulants (e.g., Warfarin)
Concomitant use with antibiotics including carbapenems has been reported to increase anticoagulant effect. Regular monitoring of INR is advised.
Storage
Store the dry powder vials below 30°C. Protect from light and moisture. Do not freeze. Reconstituted solution should be used immediately or stored as per manufacturer's guidelines.
Overdose
In case of overdose, discontinue Merocil 250 mg Injection and institute general supportive treatment. Meropenem can be removed by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed and the potential benefit justifies the potential risk to the fetus. Meropenem is excreted in human milk; caution should be exercised when administered to a nursing woman.
