Description
Overview of the medicine
Meropenem is a broad-spectrum carbapenem antibiotic used to treat a variety of bacterial infections, including intra-abdominal infections, meningitis, and complicated skin and skin structure infections. It is administered intravenously.
Uses & Indications
Dosage
Adults
Usually 500 mg to 1 g intravenously every 8 hours, depending on the type and severity of infection. For meningitis, 1 g to 2 g every 8 hours.
Elderly
Dose adjustment based on renal function may be necessary; monitor creatinine clearance.
Renal_impairment
Dose must be reduced based on creatinine clearance (CrCl). For CrCl 26-50 mL/min: 1 dose unit every 12 hours. For CrCl 10-25 mL/min: 0.5 dose unit every 12 hours. For CrCl <10 mL/min: 0.5 dose unit every 24 hours.
How to Take
Administer intravenously as an infusion over 15-30 minutes or as a bolus injection over 3-5 minutes (for doses up to 20 mL). Reconstitute with sterile water for injection or compatible diluent.
Mechanism of Action
Meropenem is a carbapenem antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.

Rapid, typically within minutes of IV administration.
Excretion
Mainly renal, approximately 70% as unchanged drug within 12 hours.

Approximately 1 hour in adults with normal renal function.

Side Effects
Contraindications
- •Hypersensitivity to meropenem or any component of the formulation
- •Severe hypersensitivity (e.g., anaphylactic reaction, severe skin reaction) to any other beta-lactam antibacterial agent (e.g., penicillins, cephalosporins)
Drug Interactions
Probenecid
Increases the plasma concentration and prolongs the elimination half-life of meropenem by competing for active tubular secretion.
Valproic Acid
Co-administration with meropenem may significantly decrease valproic acid concentrations, potentially leading to subtherapeutic levels and loss of seizure control.
Oral Anticoagulants
May potentiate the anticoagulant effect of warfarin and other oral anticoagulants; INR monitoring recommended.
Storage
Store intact vials below 30°C, protected from light. Reconstituted solutions should be used immediately; if not, store at 2-8°C and use within a specified period (e.g., 6 hours for water, 18-24 hours for normal saline).
Overdose
In case of overdose, discontinue meropenem and initiate supportive treatment. Hemodialysis can remove meropenem and its metabolite from the body.
Pregnancy & Lactation
Category B. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Excreted in breast milk, use with caution during lactation.
