Description
Overview of the medicine
Mexclav 500 mg Tablet is a combination antibiotic used to treat a wide variety of bacterial infections. It contains amoxicillin, a penicillin-type antibiotic, and clavulanic acid, a beta-lactamase inhibitor, which helps amoxicillin work more effectively against bacteria that produce beta-lactamase.
Uses & Indications
Dosage
Adults
One 500 mg/125 mg tablet every 8 hours, or as directed by physician. For severe infections, dosage may be increased.
Elderly
Similar to adult dosage, but caution with renal impairment.
Renal_impairment
Dosage adjustment required based on creatinine clearance; generally reduced frequency or dose.
How to Take
Take orally with a glass of water, preferably at the start of a meal to minimize potential gastrointestinal intolerance. Complete the full course of treatment as prescribed, even if symptoms improve.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death. Clavulanic acid irreversibly binds to and inactivates beta-lactamase enzymes produced by bacteria, which would otherwise degrade amoxicillin, thus protecting amoxicillin from hydrolysis.

Excretion
Primarily excreted unchanged in the urine (amoxicillin) and as metabolites (clavulanic acid).
Half life
Approximately 1-1.5 hours for both amoxicillin and clavulanic acid.
Absorption
Well absorbed from the gastrointestinal tract, food can reduce absorption of clavulanic acid slightly but overall absorption is good.
Metabolism
Amoxicillin is minimally metabolized; clavulanic acid is extensively metabolized.

Contraindications
- History of hypersensitivity to penicillin or other beta-lactam antibiotics
- History of amoxicillin-clavulanate-associated jaundice/hepatic dysfunction
- Infectious mononucleosis (increased risk of rash)
Drug Interactions
Warfarin
Increased anticoagulant effect, monitor INR.
Allopurinol
Increased incidence of rash, particularly in patients with hyperuricemia.
Methotrexate
Reduced renal tubular secretion of methotrexate, potentially increasing toxicity.
Oral contraceptives
May reduce efficacy of oral contraceptives (advise additional contraception).
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include gastrointestinal upset, electrolyte imbalance, crystalluria, and convulsions. Treatment is symptomatic and supportive; hemodialysis can remove amoxicillin/clavulanic acid from circulation.
