Description
Overview of the medicine
Micogyl 200 mg Suspension contains Metronidazole, an antibiotic and antiprotozoal medication used to treat various bacterial and parasitic infections, including amoebiasis, giardiasis, trichomoniasis, and anaerobic bacterial infections.
Uses & Indications
Dosage
Adults
Dosage varies by indication. Common examples: Amoebiasis: 400-800 mg 3 times daily for 5-10 days. Giardiasis: 2 g once daily for 3 days or 250 mg 3 times daily for 5-7 days. Consult a physician for specific dosing.
Elderly
Use with caution. Dosage adjustments may be considered based on hepatic or renal function.
Children
Dosage varies by weight and indication. Common example: Amoebiasis: 35-50 mg/kg/day in 3 divided doses for 5-10 days. Always consult a pediatrician.
Renal_impairment
Generally, no dose adjustment is necessary for mild to moderate renal impairment. For severe impairment (CrCl <10 mL/min), dose reduction may be considered, but metabolites can accumulate. Consult physician.
Hepatic_impairment
Use with caution in patients with severe hepatic impairment. Dose reduction may be necessary due to reduced metabolism and prolonged half-life. Consult physician.
How to Take
Take orally, preferably with or after food, to minimize gastrointestinal upset. Shake the suspension well before each use. Use an accurate measuring device provided with the medicine.
Mechanism of Action
Metronidazole is a nitroimidazole derivative. It enters the microbial cell, where its nitro group is reduced by electron transfer proteins, forming highly reactive nitro radical anions. These free radicals disrupt DNA synthesis and cause DNA damage, leading to microbial cell death.
Pharmacokinetics
Onset
Varies by infection type and severity, typically within hours to days.
Excretion
Mainly renal (60-80% as metabolites and unchanged drug); a small amount is excreted in feces.
Half life
Approximately 8 hours (range 6-12 hours).
Absorption
Rapid and complete absorption after oral administration; peak plasma concentrations reached within 1-2 hours.
Metabolism
Primarily hepatic, by oxidation, hydroxylation, and glucuronidation, forming active metabolites.
Side Effects
Contraindications
- •Hypersensitivity to metronidazole or other nitroimidazole derivatives.
- •First trimester of pregnancy (relative contraindication, risk/benefit assessment required in later trimesters).
Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, flushing, tachycardia, headache, abdominal cramps). Avoid alcohol during treatment and for at least 48 hours after the last dose.
Lithium
Increases serum lithium levels, potentially leading to lithium toxicity. Monitor lithium levels.
Busulfan
Increases busulfan plasma concentrations, potentially leading to busulfan toxicity.
Cimetidine
May prolong metronidazole half-life and decrease plasma clearance.
Disulfiram
Psychotic reactions when co-administered. Avoid concurrent use.
Vecuronium
May prolong neuromuscular blockade.
Phenobarbital / Phenytoin
May decrease metronidazole plasma levels by inducing hepatic enzymes.
Warfarin and other oral anticoagulants
Enhances anticoagulant effect, leading to increased risk of bleeding. Monitor INR/PT closely.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, ataxia, dizziness, and disorientation. Management is symptomatic and supportive. There is no specific antidote.
Pregnancy & Lactation
Pregnancy: Contraindicated in the first trimester. Use with caution in the second and third trimesters only if clearly needed and benefits outweigh the potential risks. Consult your doctor. Lactation: Metronidazole is excreted in breast milk. Breastfeeding should be temporarily discontinued during therapy and for 12-24 hours after the last dose, or an alternative medication should be considered.
