Description
Overview of the medicine
Mig 25 mg Tablet contains Migratriptan, a selective serotonin 5-HT1B/1D receptor agonist used for the acute treatment of migraine attacks with or without aura. It works by constricting dilated blood vessels in the brain and inhibiting the release of certain chemicals that cause migraine symptoms.
Uses & Indications
Dosage
Adults
25 mg taken orally at the first sign of a migraine headache. If symptoms recur, a second dose may be taken after at least 2 hours. Maximum daily dose: 50 mg.
Elderly
Dosage adjustment not usually required, but caution is advised due to potential age-related cardiovascular risk factors. Use with care.
Renal_impairment
No dose adjustment needed for mild to moderate renal impairment. Not recommended for severe renal impairment.
How to Take
Take the tablet whole with water, with or without food, as soon as migraine symptoms appear. Do not crush, chew, or split the tablet.
Mechanism of Action
Migratriptan selectively binds to 5-HT1B and 5-HT1D receptors on intracranial blood vessels and trigeminal nerves. This binding leads to vasoconstriction of cranial blood vessels, which are dilated during a migraine attack, and inhibits the release of neuropeptides from trigeminal nerve endings, thereby reducing neurogenic inflammation and pain transmission.
Pharmacokinetics
Onset
Onset of action typically occurs within 30-60 minutes.
Excretion
Mainly excreted renally, with a smaller portion excreted via feces.
Half life
Approximately 2-3 hours.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are usually reached within 1-2 hours.
Metabolism
Primarily hepatic metabolism via cytochrome P450 enzymes (e.g., CYP1A2, CYP3A4) and other metabolic pathways.
Side Effects
Contraindications
- •Ischemic heart disease (e.g., angina pectoris, myocardial infarction)
- •Uncontrolled hypertension
- •History of stroke or transient ischemic attack (TIA)
- •Peripheral vascular disease
- •Severe hepatic impairment
- •Concurrent use with ergotamine-containing medications or other 5-HT1 agonists
- •Hypersensitivity to Migratriptan or any excipients
Drug Interactions
SSRIs/SNRIs
Potential for serotonin syndrome (agitation, hallucinations, rapid heart rate, fever, muscle rigidity, nausea, vomiting, diarrhea).
MAO Inhibitors
Increased plasma levels of Migratriptan. Concomitant use is contraindicated.
Ergotamine and Ergot-type drugs
Increased risk of vasospastic reactions. Do not use within 24 hours of each other.
Other 5-HT1 agonists (triptans)
Increased risk of cardiovascular and serotonin syndrome. Do not use within 24 hours of each other.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include severe side effects like prolonged vasospasm, hypertension, cardiovascular events, and neurological symptoms. Treatment is symptomatic and supportive, focusing on maintaining cardiovascular and respiratory function.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies the potential risk to the fetus. It is unknown if Migratriptan is excreted in human milk; use with caution in nursing mothers.
