Description
Overview of the medicine
Mirogabalin is a gabapentinoid medication used for the treatment of various forms of neuropathic pain, including postherpetic neuralgia and diabetic peripheral neuropathic pain. It works by selectively binding to the α2δ-1 subunit of voltage-gated calcium channels.
Uses & Indications
Dosage
Adults
Initial: 5 mg twice daily. Titrate up to 15-25 mg twice daily over several weeks based on response and tolerability. Max: 60 mg/day.
Elderly
Start with lower doses and titrate carefully, considering renal function.
Renal_impairment
Dose adjustment required. For moderate impairment (CrCl 30-60 mL/min), reduce dose by 50%. For severe impairment (CrCl <30 mL/min), reduce by 75%.
How to Take
Administer orally with or without food. Swallow tablets whole with water. Do not crush, chew, or split.
Mechanism of Action
Mirogabalin selectively binds to the α2δ-1 subunit of voltage-gated calcium channels in the central nervous system, thereby modulating the release of various neurotransmitters. This action is believed to reduce hyperexcitability of neurons involved in neuropathic pain pathways.
Pharmacokinetics
Onset
Typically within a few days to a week.
Excretion
Primarily renal excretion of unchanged drug.
Half life
Approximately 6-8 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is high and dose-proportional.
Metabolism
Primarily excreted unchanged in urine. Minor metabolic pathways.
Side Effects
Contraindications
- •Hypersensitivity to mirogabalin or any component of the formulation
Drug Interactions
Antacids
May slightly decrease mirogabalin absorption, but generally not clinically significant.
CNS Depressants (e.g., opioids, benzodiazepines, alcohol)
May enhance central nervous system depressant effects (drowsiness, dizziness).
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Overdose may lead to increased side effects such as severe drowsiness, dizziness, and ataxia. Management is supportive, including gastric lavage or activated charcoal if recent ingestion, and close monitoring of vital signs.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit outweighs potential risk to the fetus. Not recommended during breastfeeding as it is excreted in breast milk. Consult a doctor.
