Overview of the medicine
Mirogabalin is an alpha-2-delta ligand primarily used for the management of neuropathic pain, including diabetic peripheral neuropathic pain and postherpetic neuralgia.
Uses & Indications
Dosage
Adults
Initial dose: 5 mg once daily or 2.5 mg twice daily, increasing to 5 mg twice daily within a week, and then titrated up to a maximum of 15 mg twice daily based on individual response and tolerability. Administer with or without food.
Elderly
Start with a lower dose (e.g., 2.5 mg once daily or twice daily) and titrate slowly, considering potential age-related decrease in renal function.
Renal_impairment
Dose adjustment is necessary. For mild impairment (CrCl 60 to <90 mL/min): no adjustment needed. Moderate (CrCl 30 to <60 mL/min): halve the usual dose. Severe (CrCl <30 mL/min): quarter the usual dose.
How to Take
Take orally with a glass of water, with or without food. Do not crush, chew, or break the tablet.
Mechanism of Action
Mirogabalin selectively binds to the alpha-2-delta-1 (α2δ-1) subunit of voltage-gated calcium channels in the central nervous system, leading to a reduction in calcium influx and subsequent modulation of neurotransmitter release, thereby exerting its analgesic effects.
