Description
Overview of the medicine
Montelon (Montelukast) is a leukotriene receptor antagonist used for the prevention and long-term treatment of asthma, relief of symptoms of seasonal and perennial allergic rhinitis, and prevention of exercise-induced bronchoconstriction.
Uses & Indications
Dosage
Adults
For asthma or allergic rhinitis: 10 mg orally once daily in the evening.
Elderly
No dosage adjustment is generally required for elderly patients.
Renal_impairment
No dosage adjustment is required in patients with renal impairment.
Hepatic_impairment
No dosage adjustment is required in patients with mild to moderate hepatic impairment. For severe hepatic impairment, data is not available.
Pediatric_2_to_5_years
4 mg chewable tablet or granule once daily in the evening for asthma or allergic rhinitis.
Pediatric_6_to_14_years
5 mg chewable tablet once daily in the evening for asthma or allergic rhinitis, or 10 mg for EIB.
Pediatric_over_15_years
Same as adult dosage (10 mg tablet).
How to Take
Take Montelon once daily in the evening. It can be taken with or without food. Chewable tablets should be chewed completely before swallowing. Granules can be mixed with a spoonful of soft food (e.g., applesauce, rice, or ice cream) or dissolved in a small amount of breast milk or formula.
Mechanism of Action
Montelukast selectively antagonizes the CysLT1 receptor. The cysteinyl leukotrienes (LTC4, LTD4, LTEE4) are potent inflammatory eicosanoids released from various cells including mast cells and eosinophils. They bind to CysLT1 receptors and cause bronchoconstriction, mucus secretion, vascular permeability, and eosinophil recruitment. By blocking these receptors, Montelukast reduces these effects.
Pharmacokinetics
Onset
Clinical effects generally seen within one day.
Excretion
Primarily excreted in the feces via bile (approximately 86%), with less than 0.2% excreted in urine.
Half life
Plasma half-life ranges from 2.7 to 5.5 hours in healthy adults.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are achieved in approximately 2 to 4 hours.
Metabolism
Extensively metabolized in the liver by cytochrome P450 enzymes (CYP2C8, CYP3A4, CYP2C9).
Side Effects
Contraindications
- Hypersensitivity to Montelukast or any component of the formulation.
- Montelukast is not indicated for use in the reversal of bronchospasm in acute asthma attacks, including status asthmaticus.
Drug Interactions
Rifampin
May decrease the plasma concentration of montelukast.
Gemfibrozil
May increase the systemic exposure of montelukast.
Phenobarbital
May decrease the AUC of montelukast by approximately 40%.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture.
