Overview of the medicine
Montelukast is a leukotriene receptor antagonist used for the prevention and long-term treatment of asthma, relief of symptoms of seasonal and perennial allergic rhinitis, and prevention of exercise-induced bronchoconstriction.
Uses & Indications
Dosage
Adults
For asthma and seasonal allergic rhinitis: Typically 10 mg once daily in the evening. This 5 mg tablet is generally for children 6-14 years, or specific adult titration under medical supervision.
Elderly
No dosage adjustment necessary.
Renal_impairment
No dosage adjustment necessary.
How to Take
Take orally once daily in the evening, with or without food. For asthma, it should be taken in the evening. For allergic rhinitis, the time of administration may be individualized.
Mechanism of Action
Montelukast selectively antagonizes the CysLT1 receptor, preventing the actions of leukotriene D4 (LTD4) and other cysteinyl leukotrienes. This inhibition reduces bronchoconstriction, inflammation, and mucus secretion in the airways.
Pharmacokinetics
Onset
Clinical improvement generally seen within 1 day for asthma; for allergic rhinitis, symptoms improve within a few days.
Excretion
Primarily excreted in the feces via bile (approximately 86%); less than 0.2% is excreted in the urine.
Half life
Approximately 2.7 to 5.5 hours in healthy young adults.
Absorption
Rapidly absorbed after oral administration. Peak plasma concentrations are achieved in approximately 2-4 hours for adults.
Metabolism
Extensively metabolized in the liver, primarily by CYP3A4 and CYP2C9, and to a lesser extent by CYP2C8.
