Description
Overview of the medicine
Mox 500 mg capsule contains Amoxicillin, a penicillin antibiotic used to treat a wide range of bacterial infections. It works by preventing the growth of bacteria.
Uses & Indications
Dosage
Adults
General dosage: 250 mg to 500 mg every 8 hours, or 500 mg to 875 mg every 12 hours, depending on the severity and type of infection. Maximum dose typically 4g/day.
Elderly
Similar to adult dosage, but caution and dose adjustment may be required in cases of renal impairment.
Renal_impairment
Dosage adjustment is necessary based on creatinine clearance. For CrCl < 30 mL/min, dose reduction or increased dosing interval is typically recommended.
How to Take
Take orally, with or without food. It is crucial to complete the entire prescribed course of treatment, even if symptoms improve, to prevent antibiotic resistance and ensure full recovery.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Within 30 minutes to 1 hour (for antibacterial effect).
Excretion
Primarily excreted unchanged by the kidneys via tubular secretion and glomerular filtration.
Half life
1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract. Bioavailability is approximately 70-90%. Absorption is not significantly affected by food.
Metabolism
Partially metabolized (about 10-25%) to inactive penicilloic acid.
Side Effects
Contraindications
- Known hypersensitivity to amoxicillin or any other penicillin antibiotic.
- History of severe hypersensitivity reaction to any other beta-lactam antibacterial agent (e.g., cephalosporins, carbapenems).
Drug Interactions
Warfarin
May enhance the anticoagulant effect; INR monitoring is recommended.
Probenecid
Increases and prolongs amoxicillin blood levels by decreasing renal tubular secretion.
Methotrexate
May increase methotrexate toxicity by reducing its renal clearance.
Oral Contraceptives
May rarely reduce the efficacy of oral contraceptives; advise additional contraceptive methods.
Storage
Store below 30°C in a dry place, protected from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include gastrointestinal upset (nausea, vomiting, diarrhea), fluid and electrolyte imbalance. In rare cases, crystalluria leading to renal failure has been reported. Treatment is symptomatic and supportive. Hemodialysis can remove amoxicillin from the circulation.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe for use during pregnancy, but should be used only if clearly needed. Amoxicillin is excreted in breast milk in small quantities; generally considered compatible with breastfeeding, but monitor the infant for potential adverse effects like diarrhea, rash, or fungal infections.

Doctor Notes
- Emphasize to patients the importance of completing the full antibiotic course to prevent bacterial resistance.
- Educate patients on potential allergic reactions, especially those with a history of penicillin allergy.
- Consider dose adjustments for elderly patients and those with impaired renal function.
