Description
Overview of the medicine
Moxacil 125 mg Suspension is an antibiotic medication containing Amoxicillin, a penicillin-type antibiotic. It is used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
Typically 250-500 mg every 8 hours, or 500-875 mg every 12 hours, depending on infection severity. For severe infections, up to 1 gram every 8 hours.
Elderly
Same as adults, but dose adjustment may be necessary in case of severe renal impairment.
Renal_impairment
Dosage reduction required based on creatinine clearance. For CrCl 10-30 mL/min, 250-500 mg every 12 hours. For CrCl <10 mL/min, 250-500 mg every 24 hours.
How to Take
Oral administration. Shake the suspension well before each use. Can be taken with or without food. For children, use a calibrated measuring spoon or oral syringe.
Mechanism of Action
Amoxicillin works by inhibiting the synthesis of bacterial cell walls, leading to bacterial lysis and death. It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which are crucial for cell wall synthesis.
Pharmacokinetics
Onset
Typically within 1-2 hours.
Excretion
Primarily excreted unchanged by the kidneys via glomerular filtration and tubular secretion. Approximately 60-75% of a dose is excreted in urine within 6 hours.
Half life
Approximately 1 to 1.5 hours in adults with normal renal function.
Absorption
Rapidly and well absorbed from the gastrointestinal tract, especially when taken with or without food. Bioavailability is approximately 70-90%.
Metabolism
Partially metabolized in the liver to inactive metabolites; approximately 10-25% is metabolized.
Side Effects
Contraindications
- Hypersensitivity to amoxicillin or any other penicillin antibiotic
- History of severe allergic reaction (e.g., anaphylaxis) to any beta-lactam antibiotic
Drug Interactions
Probenecid
Increases and prolongs plasma levels of amoxicillin.
Allopurinol
Increased risk of skin rash.
Methotrexate
Reduced renal excretion of methotrexate, potentially increasing toxicity.
Oral contraceptives
May reduce the efficacy of oral contraceptives (advise additional contraception).
Oral anticoagulants (e.g., Warfarin)
May prolong prothrombin time; monitor INR.
Storage
Store dry powder below 30°C, protected from light and moisture. Store reconstituted suspension in a refrigerator (2-8°C) and do not freeze.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and rarely, convulsions or crystalluria leading to renal failure. Management is generally supportive and symptomatic. Gastric lavage or activated charcoal may be considered soon after ingestion. Hemodialysis can remove amoxicillin from the circulation.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe for use during pregnancy, but use only if clearly needed. Amoxicillin is excreted in breast milk; use with caution in breastfeeding mothers due to potential for sensitization or gastrointestinal disturbances in the infant.

YesClinical Trials
Amoxicillin has been extensively studied since its introduction in 1972, with numerous clinical trials establishing its efficacy and safety in various bacterial infections. Ongoing research explores its role in emerging antimicrobial resistance.
Lab Monitoring
- Renal function tests (for prolonged therapy or renal impairment)
- Liver function tests (rarely, if hepatotoxicity is suspected)
- Complete blood count (for prolonged therapy)
Doctor Notes
- Always confirm patient's allergy status to penicillin before prescribing.
- Advise patients to complete the full course to prevent resistance.
- Monitor for signs of C. difficile infection if diarrhea is prolonged or severe.
Patient Guidelines
- Complete the full course of antibiotics as prescribed, even if you feel better.
- Shake the suspension well before each use.
- Use the provided measuring device for accurate dosing.
- Report any severe allergic reactions immediately.
- Do not save the suspension for future use; discard any unused portion after the recommended period (usually 7-14 days).
Missed Dose Advice
Take the missed dose as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose.
