Description
Overview of the medicine
Moxacil 500 mg capsule is a broad-spectrum antibiotic containing Amoxicillin, used to treat a variety of bacterial infections.
Uses & Indications
Dosage
Adults
250 mg to 500 mg every 8 hours, or 750 mg to 1 g every 12 hours, depending on the severity and type of infection.
Elderly
Similar to adult dosage, but renal function should be considered.
Renal_impairment
Dosage adjustment is necessary for patients with severe renal impairment (CrCl < 30 mL/min).
How to Take
Moxacil 500 mg capsule should be swallowed whole with water, with or without food. Taking it with food may reduce gastrointestinal discomfort.
Mechanism of Action
Amoxicillin works by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs) in the bacterial cell wall, leading to cell lysis and bacterial death.
Pharmacokinetics
Onset
30-60 minutes
Excretion
Primarily excreted unchanged by the kidneys (glomerular filtration and tubular secretion) into the urine.
Half life
1-1.5 hours (in individuals with normal renal function)
Absorption
Rapidly and well absorbed from the gastrointestinal tract, especially when taken on an empty stomach. Peak plasma concentrations are achieved in about 1-2 hours.
Metabolism
Limited hepatic metabolism; about 10-25% of the dose is metabolized to inactive penicilloic acid.
Side Effects
Contraindications
- Known hypersensitivity to penicillin or any component of the formulation
- History of severe allergic reaction to other beta-lactam antibiotics (e.g., cephalosporins, carbapenems)
Drug Interactions
Warfarin
May prolong prothrombin time (INR); careful monitoring is required.
Probenecid
Decreases renal tubular secretion of amoxicillin, leading to increased and prolonged plasma levels.
Methotrexate
Amoxicillin may reduce methotrexate excretion, increasing its toxicity.
Oral contraceptives
May reduce the efficacy of oral contraceptives (rarely significant, but advise additional contraception).
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms may include gastrointestinal upset (nausea, vomiting, diarrhea), fluid and electrolyte imbalance. In cases of massive overdose, renal failure and convulsions have been reported. Treatment is symptomatic and supportive; hemodialysis may be used to remove amoxicillin from circulation.
Pregnancy & Lactation
Category B. Generally considered safe during pregnancy, but should be used with caution. Excreted in breast milk in small amounts; generally considered compatible with breastfeeding, but monitor infant for potential adverse effects (e.g., diarrhea, candidiasis).
