Description
Overview of the medicine
Moxaclav 125 mg Suspension is an antibiotic combination containing Amoxicillin and Clavulanic Acid, used to treat a wide range of bacterial infections. Amoxicillin is a penicillin-type antibiotic that fights bacteria, while Clavulanic Acid prevents bacteria from destroying Amoxicillin.
Uses & Indications
Dosage
Adults
Not typically recommended for adults in this strength; higher strengths usually prescribed. Refer to product leaflet for specific adult dosages for other forms.
Elderly
Not typically recommended for elderly in this strength; higher strengths usually prescribed. Dosage adjustment may be required based on renal function.
Renal_impairment
Dosage adjustment required. For children with creatinine clearance >30 mL/min, no adjustment. For 10-30 mL/min, 125 mg/31.25 mg every 12 hours. For <10 mL/min, 125 mg/31.25 mg every 24 hours. Hemodialysis: 125 mg/31.25 mg once daily, plus an additional dose during and at the end of dialysis.
How to Take
Oral suspension. Shake the bottle well before each use. Take with food to minimize potential gastrointestinal intolerance. Complete the full course of treatment as prescribed, even if symptoms improve.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis. Clavulanic Acid is a beta-lactamase inhibitor that inactivates a wide range of beta-lactamase enzymes produced by bacteria, thereby protecting Amoxicillin from degradation and extending its antibacterial spectrum.
Pharmacokinetics
Onset
Antibacterial effect typically observed within 30-60 minutes.
Excretion
Primarily excreted unchanged in the urine by glomerular filtration and tubular secretion (amoxicillin) and via renal and non-renal mechanisms (clavulanic acid).
Half life
Amoxicillin: Approximately 1-1.5 hours. Clavulanic Acid: Approximately 1 hour.
Absorption
Both amoxicillin and clavulanic acid are well absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are reached approximately 1-2 hours after dosing.
Metabolism
Amoxicillin is minimally metabolized; clavulanic acid is extensively metabolized.
Side Effects
Contraindications
- History of severe hypersensitivity reaction to beta-lactams (e.g., anaphylaxis to penicillin or cephalosporin)
- History of amoxicillin/clavulanic acid-associated jaundice or hepatic dysfunction
Drug Interactions
Probenecid
Increases amoxicillin blood levels and prolongs its effect.
Methotrexate
Increases methotrexate toxicity.
Oral contraceptives
May reduce the effectiveness of oral contraceptives.
Warfarin and other oral anticoagulants
May prolong prothrombin time; monitor INR closely.
Storage
Store dry powder below 25°C in a dry place, away from direct sunlight. Once reconstituted, store in a refrigerator (2-8°C) and discard any unused portion after 7-10 days.
Overdose
Overdose symptoms include gastrointestinal discomfort (abdominal pain, vomiting, diarrhea), rash, or hyperkinesis. Treatment is symptomatic and supportive. Hemodialysis can remove amoxicillin and clavulanic acid from the circulation.
Pregnancy & Lactation
Category B in pregnancy. Use with caution. Small amounts of both amoxicillin and clavulanic acid are excreted into breast milk. Generally considered safe, but monitor for potential adverse effects in the infant (e.g., diarrhea, candidiasis). Consult a doctor.
