Description
Overview of the medicine
Moxigen 400 mg Tablet contains Moxifloxacin, a broad-spectrum fluoroquinolone antibiotic used to treat a wide range of bacterial infections.
Uses & Indications
Dosage
Adults
400 mg once daily for 5-21 days depending on the infection type and severity.
Elderly
No dosage adjustment typically required based on age alone. Monitor renal function.
Renal_impairment
No dosage adjustment required for mild to moderate renal impairment. For severe renal impairment or end-stage renal disease (ESRD) including patients on dialysis, no dosage adjustment is considered necessary.
How to Take
Take orally, with or without food. Swallow the tablet whole with water. Do not crush, cut, or chew.
Mechanism of Action
Moxifloxacin inhibits bacterial DNA gyrase (topoisomerase II) and topoisomerase IV, enzymes essential for DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA processes and ultimately bacterial cell death.
Pharmacokinetics
Onset
Within 1-2 hours for therapeutic effect.
Excretion
Excreted as unchanged drug and metabolites in feces (approximately 60%) and urine (approximately 40%).
Half life
Approximately 11-15 hours.
Absorption
Well absorbed from the gastrointestinal tract; absolute bioavailability is approximately 90%. Peak plasma concentrations are achieved 0.5-4 hours after administration.
Metabolism
Undergoes hepatic metabolism mainly via glucuronide and sulfate conjugation, without involvement of cytochrome P450 system.
Side Effects
Contraindications
- Hypersensitivity to moxifloxacin or other quinolones.
- History of tendinitis or tendon rupture associated with fluoroquinolone use.
- QTc prolongation or risk factors for QTc prolongation (e.g., uncorrected hypokalemia, significant bradycardia, recent myocardial infarction, concomitant use of Class IA or III antiarrhythmics).
- Children and adolescents below 18 years of age.
- Pregnancy and lactation (unless benefits outweigh risks).
Drug Interactions
Warfarin
Enhanced anticoagulant effect, monitor INR.
Corticosteroids
Increased risk of tendon rupture.
Antacids (containing Mg/Al), Sucralfate, Iron/Zinc salts
Reduced absorption of moxifloxacin; take moxifloxacin at least 4 hours before or 8 hours after these agents.
QT-prolonging drugs (e.g., Class IA & III antiarrhythmics, tricyclic antidepressants, macrolides)
Increased risk of QTc prolongation; avoid co-administration.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
In case of acute overdose, gastric emptying and general supportive measures are recommended. Monitor ECG for QTc prolongation and provide symptomatic treatment.
Pregnancy & Lactation
Not recommended during pregnancy and lactation due to potential risks to the fetus/infant (cartilage damage, QTc prolongation). Use only if potential benefits justify the potential risks.

Lab Monitoring
- Baseline and periodic ECG monitoring (especially in patients at risk for QTc prolongation)
- Liver function tests (if hepatotoxicity suspected)
- Renal function (if prolonged use or existing impairment)
- INR if co-administered with warfarin
Doctor Notes
- Counsel patients on potential serious adverse events including tendinitis/rupture, CNS effects, and peripheral neuropathy. Advise immediate discontinuation and contact if these occur.
- Be mindful of QTc prolongation risk, especially with concomitant medications or pre-existing conditions.
Patient Guidelines
- Complete the full course of treatment as prescribed, even if symptoms improve.
- Drink plenty of fluids to avoid crystalluria.
- Avoid excessive exposure to sunlight or artificial UV light.
- Report any tendon pain, swelling, numbness, or tingling immediately to your doctor.
Missed Dose Advice
If a dose is missed, take it as soon as you remember, unless it is almost time for your next dose. Do not double the dose to make up for a missed one.
