Description
Overview of the medicine
Moxquin 400 mg Tablet contains Moxifloxacin, a broad-spectrum fluoroquinolone antibiotic, used to treat various bacterial infections including respiratory tract infections, skin and soft tissue infections, and intra-abdominal infections.
Uses & Indications
Dosage
Adults
400 mg once daily for 5-21 days, depending on the infection type and severity. Must complete the full course as prescribed.
Elderly
No dosage adjustment is generally required for elderly patients, but caution is advised in those with pre-existing cardiac conditions or electrolyte imbalances.
Renal_impairment
No dosage adjustment is necessary for patients with mild to severe renal impairment (including those on dialysis).
How to Take
Take the tablet orally, with or without food. Swallow whole with water, do not crush or chew. It is recommended to take it at approximately the same time each day.
Mechanism of Action
Moxifloxacin exerts its bactericidal action by inhibiting bacterial DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication, transcription, repair, and recombination. This leads to disruption of bacterial DNA and cell death.
Pharmacokinetics
Onset
Within hours of administration.
Excretion
Excreted in both feces (approximately 60%) and urine (approximately 40%), partially as unchanged drug and partially as metabolites.
Half life
Approximately 11-15 hours.
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract; absolute bioavailability is approximately 90%. Peak plasma concentrations are reached within 0.5-4 hours.
Metabolism
Undergoes Phase II biotransformation (glucuronide and sulfate conjugation) primarily in the liver. It is not metabolized by cytochrome P450 enzymes.
Side Effects
Contraindications
- Hypersensitivity to moxifloxacin or other quinolone antibiotics.
- History of QT prolongation or uncorrected hypokalemia.
- Patients with liver impairment (Child-Pugh C) or elevated transaminases.
- History of tendon disorders related to quinolone use.
Drug Interactions
Warfarin
May enhance the anticoagulant effect of warfarin, requiring careful monitoring of INR.
Corticosteroids
Increased risk of tendon disorders, especially in elderly patients.
Antacids, Sucralfate, Iron/Zinc supplements, Multivitamins
These agents can chelate moxifloxacin, significantly reducing its absorption. Administer moxifloxacin at least 4 hours before or 8 hours after these products.
QT prolonging drugs (e.g., antiarrhythmics, tricyclic antidepressants, macrolides, antipsychotics)
Concomitant use may increase the risk of QT interval prolongation and arrhythmias. Avoid co-administration.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.

In case of overdose, symptomatic treatment and supportive measures are recommended. There is no specific antidote. ECG monitoring and blood pressure monitoring are important. Activated charcoal may be useful shortly after ingestion.
Pregnancy & Lactation
Moxifloxacin should not be used during pregnancy or lactation unless the potential benefit justifies the potential risk to the fetus or infant. Animal studies have shown adverse effects on bone and cartilage development. Excreted in breast milk.
