Description
Overview of the medicine
Amoxicillin is a penicillin-type antibiotic used to treat a wide variety of bacterial infections.
Uses & Indications
Dosage
Adults
Standard adult dose for mild-to-moderate infections: 250-500 mg every 8 hours or 500-875 mg every 12 hours. For severe infections, dosage may be increased. Usually for 7-10 days.
Elderly
No specific dosage adjustment for elderly based on age alone, but renal function should be assessed and dose adjusted if impaired.
Renal_impairment
Dosage reduction is required. For creatinine clearance <30 mL/min, 250-500 mg every 12 hours. Patients on hemodialysis should receive a supplemental dose after dialysis.
How to Take
Take orally with or without food. Swallow the capsule whole with water. Do not crush or chew.
Mechanism of Action
Amoxicillin inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to bacterial cell lysis and death.
Pharmacokinetics
Onset
Peak plasma concentrations are achieved within 1-2 hours.
Excretion
Primarily excreted unchanged in the urine (50-70%) via renal tubular secretion and glomerular filtration within 6 hours.
Half life
Approximately 1-1.5 hours in adults with normal renal function.
Absorption
Rapidly and well-absorbed (70-90% bioavailability) after oral administration; food does not significantly affect absorption.
Metabolism
Partially metabolized in the liver to inactive penicilloic acid.
Side Effects
Contraindications
- Known hypersensitivity to penicillins or any component of the formulation.
- History of a severe immediate hypersensitivity reaction (e.g., anaphylaxis, Stevens-Johnson syndrome) to any other beta-lactam antibacterial agent (e.g., cephalosporins, carbapenems).
Drug Interactions
Probenecid
Concurrent use of probenecid decreases the renal tubular secretion of amoxicillin, resulting in increased and prolonged blood levels of amoxicillin.
Methotrexate
Amoxicillin may reduce the renal excretion of methotrexate, potentially increasing methotrexate toxicity.
Oral Contraceptives
Amoxicillin may reduce the efficacy of oral contraceptives in some cases. Additional birth control methods may be advisable.
Oral Anticoagulants (e.g., Warfarin)
May prolong prothrombin time (INR) in patients receiving oral anticoagulants. Careful monitoring is advised.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include nausea, vomiting, diarrhea, and crystalluria. Management involves symptomatic and supportive treatment. Gastric lavage may be considered. Amoxicillin can be removed from circulation by hemodialysis.
Pregnancy & Lactation
Pregnancy Category B. Generally considered safe during pregnancy. Amoxicillin is excreted in small amounts in breast milk; generally considered compatible with breastfeeding, but monitor infant for potential side effects (e.g., diarrhea, candidiasis).

Lab Monitoring
- For prolonged therapy, periodic assessment of renal, hepatic, and hematopoietic function is recommended.
- Monitor INR in patients on oral anticoagulants.
Doctor Notes
- Emphasize to patients the importance of completing the full course of therapy to prevent relapse and resistance.
- Advise patients to report any signs of allergic reactions immediately.
- Consider renal function when dosing, especially in elderly patients or those with known renal impairment.
Patient Guidelines
- Complete the entire course of medication as prescribed, even if symptoms improve, to prevent recurrence and development of antibiotic resistance.
- Do not share this medication with others, as it may be ineffective or harmful for their condition.
- Consult your doctor if symptoms do not improve or worsen after a few days of treatment.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and resume your regular dosing schedule. Do not double the dose to catch up.
Driving Precautions
