Description
Overview of the medicine
Naspro Plus is a combination medicine containing Naproxen, a non-steroidal anti-inflammatory drug (NSAID), and Esomeprazole, a proton pump inhibitor (PPI). It is used to relieve pain, inflammation, and stiffness associated with osteoarthritis, rheumatoid arthritis, and ankylosing spondylitis while reducing the risk of stomach ulcers often caused by NSAIDs.
Uses & Indications
Dosage
Adults
One tablet (Naproxen 500 mg + Esomeprazole 20 mg) orally twice daily, preferably at least 30 minutes before meals.
Elderly
Consider lower doses or less frequent administration, monitor for adverse effects.
Renal_impairment
Use with caution; dose adjustment may be necessary in patients with moderate to severe renal impairment. Avoid in severe renal failure.
How to Take
Take the tablet whole with water. Do not crush, chew, or split the tablet. It should be taken at least 30 minutes before a meal for optimal Esomeprazole absorption and gastric protection, though it may also be taken with or after food to minimize stomach upset from Naproxen.
Mechanism of Action
Naproxen inhibits cyclooxygenase (COX-1 and COX-2) enzymes, reducing prostaglandin synthesis, thereby producing anti-inflammatory, analgesic, and antipyretic effects. Esomeprazole is a proton pump inhibitor that suppresses gastric acid secretion by inhibiting the H+/K+-ATPase in gastric parietal cells, providing gastroprotection.
Pharmacokinetics
Onset
Naproxen: Pain relief within 1-2 hours. Esomeprazole: Acid suppression within 1 hour.
Excretion
Naproxen and its metabolites are primarily excreted in the urine. Esomeprazole metabolites are primarily excreted in the urine and to a lesser extent in the feces.
Half life
Naproxen: 12-17 hours. Esomeprazole: Approximately 1-1.5 hours.
Absorption
Naproxen is rapidly and completely absorbed from the GI tract. Esomeprazole is rapidly absorbed after oral administration.
Metabolism
Both Naproxen and Esomeprazole are primarily metabolized in the liver via cytochrome P450 (CYP) enzymes, mainly CYP2C9 and CYP1A2 for Naproxen, and CYP2C19 and CYP3A4 for Esomeprazole.
Side Effects
Contraindications

Increased digoxin levels.
Lithium
Increased lithium plasma concentrations.
Phenytoin
Increased phenytoin levels.
Clopidogrel
Reduced antiplatelet effect of clopidogrel due to CYP2C19 inhibition by esomeprazole.
Methotrexate
Increased methotrexate toxicity.
Corticosteroids
Increased risk of GI ulceration or bleeding.
Diuretics (e.g., Furosemide)
Reduced diuretic and antihypertensive effects.
Anticoagulants (e.g., Warfarin)
Increased risk of bleeding.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of Naproxen overdose may include drowsiness, heartburn, nausea, vomiting, epigastric pain, GI bleeding. Esomeprazole overdose symptoms are generally mild. Management is symptomatic and supportive. Gastric lavage and/or activated charcoal may be considered within a few hours of ingestion. Hemodialysis is generally not effective for Naproxen.
Pregnancy & Lactation
Avoid in the third trimester of pregnancy due to potential for premature closure of the fetal ductus arteriosus. Use with caution during early pregnancy and lactation only if clearly needed and the potential benefits outweigh the risks. Consult a doctor.
