Description
Overview of the medicine
Neforex (Fexofenadine) is an antihistamine used to relieve allergy symptoms such as watery eyes, runny nose, itching eyes/nose, sneezing, and hives. It is a non-drowsy antihistamine.
Uses & Indications
Dosage
Adults
For seasonal allergic rhinitis: 120 mg or 180 mg once daily. For chronic idiopathic urticaria: 180 mg once daily.
Elderly
No specific dose adjustment is recommended for elderly patients with normal renal function. Close monitoring is advised.
Renal_impairment
For patients with impaired renal function (creatinine clearance < 80 mL/min), the recommended starting dose is 60 mg once daily for both seasonal allergic rhinitis and chronic idiopathic urticaria.
How to Take
Neforex tablets should be taken orally with water. It can be taken with or without food. Avoid taking with fruit juices (grapefruit, orange, apple) as they may reduce the absorption of fexofenadine.
Mechanism of Action
Fexofenadine is a selective peripheral H1-receptor antagonist. It prevents the effects of histamine by blocking its action on H1 receptors, thereby relieving allergy symptoms without causing significant drowsiness due to its minimal penetration into the central nervous system.
Pharmacokinetics
Onset
Onset of action within 1 hour.
Excretion
Primarily excreted in feces (approximately 80%) and urine (approximately 11%) as unchanged drug.
Half life
Plasma elimination half-life is approximately 11-15 hours.
Absorption
Rapidly absorbed after oral administration, with peak plasma concentrations reached in 1-3 hours. Absolute bioavailability is unknown.
Metabolism
Undergoes negligible hepatic metabolism; about 5% of the total dose is metabolized.
Side Effects
Contraindications
- Known hypersensitivity to fexofenadine hydrochloride or any component of the formulation.
- Children under 6 years of age for seasonal allergic rhinitis (for 60 mg tablet) and children under 12 years for 120 mg and 180 mg tablets.
Drug Interactions
Erythromycin and Ketoconazole
Co-administration with erythromycin or ketoconazole has been shown to result in increased plasma levels of fexofenadine. However, these increases were not associated with any increase in adverse events or prolongation of the QTc interval.
Aluminum and Magnesium-containing antacids
Concomitant administration of antacids containing aluminum and magnesium hydroxide may reduce the absorption of fexofenadine. It is advisable to separate the administration by approximately 2 hours.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose may include drowsiness, dizziness, and dry mouth. In case of overdose, symptomatic and supportive measures are recommended. Hemodialysis does not effectively remove fexofenadine from blood.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Fexofenadine is excreted in human breast milk; therefore, caution should be exercised when Neforex is administered to a nursing woman.
