Description
Overview of the medicine
Neugalin 75 mg capsule contains Pregabalin, which is used to treat neuropathic pain, fibromyalgia, partial-onset seizures, and generalized anxiety disorder.
Uses & Indications
Dosage
Adults
Dosage varies by indication. Typically initiated at 75 mg twice daily or 50 mg three times daily, adjusted up to a maximum of 300 mg twice daily or 200 mg three times daily (total 600 mg/day).
Elderly
May require dose reduction due to age-related decrease in renal function. Initial dose typically 25-50 mg three times daily, adjusted based on response and tolerability.
Renal_impairment
Requires significant dose adjustment. For CrCl 30-60 mL/min, 25-75 mg BID; CrCl 15-30 mL/min, 25-50 mg once daily or 25 mg BID; CrCl <15 mL/min, 25 mg once daily or 25 mg every other day. Hemodialysis patients require a supplemental dose.
How to Take
Take orally with or without food. Swallow the capsule whole with water. Do not crush, chew, or open the capsule.
Mechanism of Action
Pregabalin binds with high affinity to the alpha2-delta subunit of voltage-gated calcium channels in the central nervous system, which is thought to reduce the release of several neurotransmitters, including glutamate, norepinephrine, and substance P. This action modulates neuronal excitability, leading to its anticonvulsant, anxiolytic, and analgesic effects.
Pharmacokinetics
Onset
Analgesic effect observed within 1 week; anxiolytic effect within 1 week, seizure control varies.
Excretion
Primarily excreted unchanged in the urine (98% of the dose).
Half life
Approximately 6.3 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations occur within 1.5 hours.
Metabolism
Negligibly metabolized in humans; less than 0.1% of the dose is recovered as metabolites in urine.
Side Effects
Contraindications
- Known hypersensitivity to Pregabalin or any component of the formulation.
Drug Interactions
Thiazolidinediones (e.g., pioglitazone, rosiglitazone)
May increase the risk of weight gain and peripheral edema when co-administered with Pregabalin.
CNS Depressants (e.g., alcohol, benzodiazepines, opioids)
May potentiate the sedative effects and respiratory depression.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, confusion, agitation, and restlessness. Treatment is supportive and symptomatic; hemodialysis may be considered in severe cases as Pregabalin is dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. Pregabalin is excreted into human milk; breastfeeding is not recommended unless benefits outweigh risks.

Lab Monitoring
- Monitor renal function (creatinine clearance) periodically, especially in elderly and those with pre-existing renal impairment. Monitor for weight gain and edema.
Doctor Notes
- Counsel patients on potential for dizziness and somnolence. Advise against abrupt discontinuation due to withdrawal risk. Monitor for suicidal ideation.
- Consider dose reduction in renal impairment and elderly patients.
Patient Guidelines
- Do not stop taking Pregabalin abruptly; withdrawal symptoms may occur. Consult your doctor for gradual dose reduction.
- Avoid alcohol and other CNS depressants while taking this medicine.
- Report any unusual changes in mood or behavior, or suicidal thoughts, to your doctor immediately.
