Description
Overview of the medicine
Nexum 20 mg Capsule is a proton pump inhibitor (PPI) used to reduce the amount of acid produced in the stomach. It treats conditions such as gastroesophageal reflux disease (GERD), ulcers, and other acid-related disorders by blocking the action of proton pumps in the stomach lining.
Uses & Indications
Dosage
Adults
For most indications (e.g., GERD, erosive esophagitis), 20 mg or 40 mg once daily for 4-8 weeks. For H. pylori eradication, 20 mg twice daily for 10 days in combination therapy.
Elderly
No dosage adjustment is generally required for elderly patients, but caution should be exercised.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Take the capsule whole, at least 30 minutes before a meal, with a glass of water. Do not chew or crush the capsule.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells. This irreversible inhibition blocks the final step in gastric acid secretion, significantly reducing gastric acidity.
Pharmacokinetics
Onset
Acid secretion inhibition begins within 1 hour; full therapeutic effect typically observed within 2-3 days of daily dosing.
Excretion
Approximately 80% of metabolites are renally excreted, with the remaining 20% excreted in feces.
Half life
The plasma elimination half-life is approximately 1.3 hours.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations reached in 1-1.6 hours. Bioavailability is approximately 64% after a single dose, increasing to 89% after repeated once-daily dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system, specifically CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Known hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Warfarin
Increased INR and prothrombin time, increasing risk of bleeding.
Tacrolimus
May increase plasma concentrations of tacrolimus.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel.
Methotrexate
May increase methotrexate levels and extend its elimination time.
Atazanavir/Nelfinavir
Significant reduction in plasma levels of these antiretrovirals, reducing their efficacy.
Storage
Store below 30°C in a dry place, protected from moisture and light. Keep out of reach of children.
Overdose
In case of overdose, treatment should be symptomatic and supportive. No specific antidote is known.
Pregnancy & Lactation
Pregnancy Category C (older classification, current data suggests low risk). Use during pregnancy only if clearly needed. Esomeprazole is excreted in breast milk; use with caution in lactating mothers.
