Nexum 40 mg capsule contains Esomeprazole, a proton pump inhibitor used to reduce stomach acid production and treat acid-related conditions.
Uses & Indications
Dosage
Adults
For GERD/Erosive Esophagitis: 20-40 mg once daily for 4-8 weeks. For Zollinger-Ellison Syndrome: 40-160 mg once daily, individualized. For H. pylori eradication: 20 mg twice daily for 7-14 days with appropriate antibiotics.
Elderly
No specific dosage adjustment is generally required, but caution is advised in severe liver impairment.
Renal_impairment
No dosage adjustment is necessary for patients with renal impairment.
How to Take
Swallow capsule whole with water at least one hour before a meal. Do not chew or crush. If swallowing is difficult, open capsule, mix contents with a small amount of water, and drink immediately.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase enzyme in gastric parietal cells, thereby reducing gastric acid secretion.
Pharmacokinetics
Onset
Within 1 hour (acid suppression).
Excretion
Primarily excreted in urine (80%) and feces (20%) as metabolites.
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is about 64% (single dose) to 89% (repeated dose). Peak plasma concentration reached in 1-1.5 hours.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system (primarily CYP2C19 and CYP3A4).
Side Effects
Contraindications
- •Hypersensitivity to Esomeprazole or other substituted Benzimidazoles
- •Concomitant use with Nelfinavir
Drug Interactions
Digoxin
Increased Digoxin levels due to increased gastric pH.
Warfarin
Potential for increased INR/prothrombin time; monitor closely.
Nelfinavir
Decreased Nelfinavir levels due to increased gastric pH.
Clopidogrel
Reduced antiplatelet effect of Clopidogrel (conflicting data, monitor patient).
Methotrexate
Increased Methotrexate levels; avoid concomitant use or monitor closely.
Storage
Store below 30°C in a dry place, away from direct sunlight and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and include confusion, drowsiness, blurred vision, tachycardia, nausea, sweating, headache, dry mouth, and other gastrointestinal disturbances. Treatment is symptomatic and supportive. Esomeprazole is highly protein-bound and not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Excreted in breast milk, caution advised during lactation.
