Description
Overview of the medicine
Esomeprazole is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. Nexum 40 mg injection is used for short-term treatment of Gastroesophageal Reflux Disease (GERD) with erosive esophagitis, as an alternative to oral therapy when oral administration is not appropriate, or for the treatment of Zollinger-Ellison syndrome and other pathological hypersecretory conditions.
Uses & Indications
Dosage
Adults
For erosive esophagitis, 20 mg or 40 mg IV once daily for 7 to 10 days. For Zollinger-Ellison syndrome, initially 80 mg IV twice daily, adjusted as needed. Administer via intravenous infusion over 10 to 30 minutes.
Elderly
No specific dosage adjustment is generally required, but caution is advised due to potential age-related decrease in liver function.
Renal_impairment
No dosage adjustment is generally required for patients with impaired renal function.
How to Take
Administer Nexum 40 mg injection as an intravenous infusion over a period of 10 to 30 minutes. Do not administer as a bolus injection. The powder must be reconstituted with the provided solvent before use.
Mechanism of Action
Esomeprazole is a proton pump inhibitor that specifically inhibits the H+/K+-ATPase (proton pump) in the gastric parietal cells. By irreversibly binding to and inhibiting this enzyme, it blocks the final step in gastric acid secretion, thereby reducing acid production.

Onset
Acid secretion inhibition occurs within 30 minutes of administration.
Excretion
Approximately 80% is excreted in the urine and 20% in the feces.
Half life
Approximately 1.2 to 1.5 hours.
Absorption
Rapidly absorbed after intravenous administration, with 100% bioavailability.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 enzyme system, primarily CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Known hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir (an antiretroviral drug).
Drug Interactions
Warfarin
Monitor INR/prothrombin time; esomeprazole may increase systemic exposure to warfarin.
Clopidogrel
Reduced effectiveness of clopidogrel due to inhibition of CYP2C19, which is crucial for its activation. Consider alternative antiplatelet therapy.
Methotrexate
May increase serum levels of methotrexate, particularly in high-dose therapy; consider temporary withdrawal of esomeprazole.
Nelfinavir, Atazanavir
Decreased absorption of antiretrovirals due to increased gastric pH, potentially reducing their effectiveness.
Storage
Store below 25°C. Protect from light and moisture. Do not freeze. Reconstituted solution should be used within 6 hours if stored at room temperature or within 24 hours if refrigerated.
Overdose
Limited experience with esomeprazole overdose. Symptoms are generally mild and include gastrointestinal disturbances and fatigue. Treatment should be symptomatic and supportive. Esomeprazole is not readily dialyzable.
Pregnancy & Lactation
Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Esomeprazole is excreted in human milk; caution is advised when administering to a nursing woman.
