Overview of the medicine
Nidazyl 200 mg Suspension contains Metronidazole, an antibiotic and antiprotozoal medication used to treat a wide variety of infections, particularly those caused by anaerobic bacteria and certain parasites.
Uses & Indications
Dosage
Adults
Typically 400-800 mg three times daily depending on the infection. For amoebiasis, 400-800 mg three times daily for 5-10 days. For giardiasis, 250-500 mg three times daily for 5-7 days. For anaerobic infections, usually 500 mg every 8 hours.
Elderly
Lower doses may be considered for elderly patients, especially with impaired hepatic function. Consult a physician.
Renal_impairment
No dosage adjustment needed for mild to moderate renal impairment. For severe renal impairment or dialysis patients, a reduced dose or extended dosing interval may be required.
How to Take
Shake well before use. Take orally with or without food. To minimize gastrointestinal upset, it is often recommended to take with food.
Mechanism of Action
Metronidazole enters bacterial/protozoal cells, where its nitro group is reduced by electron transport proteins, forming highly reactive nitroso free radicals. These radicals damage DNA and other biomolecules, leading to cell death.

Therapeutic effects typically seen within 24-48 hours.
Excretion
Primarily excreted in the urine (60-80% as metabolites and unchanged drug), with a smaller portion excreted in the feces (6-15%).
Half life
Approximately 8 hours (range 6-12 hours).
Absorption
Rapidly and almost completely absorbed from the gastrointestinal tract after oral administration. Peak plasma concentrations are achieved within 1-2 hours.
Metabolism
Extensively metabolized in the liver by hydroxylation and glucuronidation. The main metabolite, 2-hydroxymethyl metronidazole, also has antibacterial activity.
Side Effects
Contraindications

Drug Interactions
Alcohol
Disulfiram-like reaction (nausea, vomiting, flushing, tachycardia, headache).
Lithium
Increases serum lithium levels, leading to toxicity.
Warfarin
Potentiates anticoagulant effect, increasing bleeding risk. Monitor INR closely.
Disulfiram
Psychotic reactions.
Storage
Store below 30°C. Protect from light and moisture. Do not freeze. Keep out of reach of children.
Overdose
Symptoms may include nausea, vomiting, ataxia, and mild disorientation. There is no specific antidote. Management is supportive and symptomatic.
Pregnancy & Lactation
Pregnancy Category B (FDA). Avoid during the first trimester if possible. Use with caution during the second and third trimesters if clearly needed. Metronidazole is excreted in breast milk; a decision should be made whether to discontinue nursing or the drug, taking into account the importance of the drug to the mother.
