Description
Overview of the medicine
Metronidazole is an antibiotic and antiprotozoal medication used to treat a wide variety of infections caused by anaerobic bacteria and certain parasites. The injection form is typically used for severe infections or when oral administration is not feasible.
Uses & Indications
Dosage
Adults
Initial dose 15 mg/kg IV over 1 hour, followed by 7.5 mg/kg IV every 6 hours or 500 mg every 8 hours, infused over 30-60 minutes, for 7-10 days or longer depending on the infection. Max: 4 g/day.
Elderly
No specific dose adjustment, but use with caution in patients with hepatic impairment.
Renal_impairment
Dosage adjustment generally not required in mild to moderate renal impairment. For severe renal impairment (CrCl <10 mL/min) or in patients undergoing hemodialysis, dose reduction or extended intervals may be considered.
How to Take
Administer intravenously only by slow infusion over 30-60 minutes. Do not give as an IV bolus.
Mechanism of Action
Metronidazole enters susceptible microbial cells where its nitro group is reduced by electron transport proteins, forming reactive nitroso free radicals. These radicals damage microbial DNA and proteins, leading to cell death.

Onset
Rapid, within minutes of IV infusion.
Excretion
Mainly renal (60-80%), with a small amount excreted in feces (6-15%).
Half life
Approximately 6-8 hours.
Absorption
Rapid and complete systemic absorption after intravenous administration.
Metabolism
Primarily hepatic via oxidation and glucuronidation. Major metabolite is 2-hydroxymetronidazole.
Side Effects
Contraindications
- •Hypersensitivity to metronidazole or other nitroimidazole derivatives.
- •First trimester of pregnancy (unless absolutely necessary).
Drug Interactions
Alcohol
Disulfiram-like reaction (abdominal cramps, nausea, vomiting, flushing, headache). Avoid alcohol during and for at least 3 days after treatment.
Lithium
May increase serum lithium levels, leading to toxicity. Monitor lithium levels.
Warfarin
Potentiates anticoagulant effect, increasing risk of bleeding. Monitor INR.
Cimetidine
May increase metronidazole levels by decreasing its metabolism.
Disulfiram
Concurrent use may cause psychotic reactions. Do not administer metronidazole within 14 days of disulfiram.
Phenobarbital/Phenytoin
May decrease metronidazole levels by increasing its metabolism.
Storage
Store below 30°C. Protect from light. Do not freeze.
Overdose
Symptoms of overdose include nausea, vomiting, ataxia, seizures, and peripheral neuropathy. There is no specific antidote; treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Avoid during the first trimester. Use with caution in later trimesters only if clearly needed. Metronidazole is excreted into breast milk; breastfeeding should be interrupted for 12-24 hours after a single dose, or an alternative should be considered for prolonged treatment.
