Description
Overview of the medicine
Noxarin (Enoxaparin Sodium) is a low molecular weight heparin (LMWH) used to prevent and treat various thromboembolic disorders, such as deep vein thrombosis (DVT) and pulmonary embolism (PE), and in acute coronary syndromes.
Uses & Indications
Dosage
Adults
For DVT prophylaxis: 40 mg subcutaneously once daily. For DVT treatment: 1 mg/kg subcutaneously every 12 hours or 1.5 mg/kg once daily.
Elderly
No dose adjustment usually required unless severe renal impairment.
Renal_impairment
For creatinine clearance < 30 mL/min, dose reduction is recommended. For example, 20 mg once daily for DVT prophylaxis.
How to Take
Administer by deep subcutaneous injection, alternately in the left or right anterolateral or posterolateral abdominal wall. Do not inject intramuscularly.
Mechanism of Action
Enoxaparin potentiates the action of antithrombin III, primarily inhibiting Factor Xa and, to a lesser extent, Factor IIa (thrombin). This inhibits clot formation.
Pharmacokinetics
Onset
Rapid onset after subcutaneous administration.
Excretion
Mainly excreted renally (approximately 40% as active fragments).
Half life
Elimination half-life of anti-Xa activity is approximately 4-5 hours.
Absorption
Subcutaneous bioavailability is approximately 100%. Peak anti-Xa activity is usually observed 3-5 hours post-dose.
Metabolism
Primarily hepatic biotransformation to inactive metabolites.
Side Effects
Contraindications
- •Active major bleeding or conditions with high risk of uncontrolled hemorrhage.
- •History of immune-mediated heparin-induced thrombocytopenia (HIT) within the past 100 days or in the presence of circulating antibodies.
- •Hypersensitivity to enoxaparin, heparin, or pork products.
- •Spinal/epidural anesthesia in patients on therapeutic enoxaparin.
Drug Interactions
Thrombolytics
Increased risk of bleeding.
Other Anticoagulants (e.g., Warfarin, Dabigatran)
Increased risk of bleeding.
Antiplatelet agents (e.g., Aspirin, Clopidogrel, NSAIDs)
Increased risk of bleeding.
Storage
Store below 25-30°C (77-86°F), protect from light and freezing. Do not refrigerate.
Overdose
Accidental overdose may lead to hemorrhagic complications. The anticoagulant effect can be largely neutralized by the slow intravenous injection of protamine sulfate.
Pregnancy & Lactation
Pregnancy Category B. Use with caution during pregnancy if clearly needed. Enoxaparin is not excreted into breast milk.
