Overview of the medicine
Numira 10 mg Tablet contains memantine, which is used to treat moderate to severe Alzheimer's disease. It helps improve memory, awareness, and the ability to perform daily functions by affecting certain brain chemicals.
Uses & Indications
Dosage
Adults
Initially 5 mg once daily, gradually increasing by 5 mg per week to a target maintenance dose of 10 mg twice daily (20 mg/day). Doses should be taken at the same time each day.
Elderly
Same as adults, with careful monitoring of renal function.
Renal_impairment
For moderate impairment (CrCl 30-49 mL/min), maximum dose 10 mg/day. For severe impairment (CrCl < 30 mL/min), maximum dose 5 mg/day.
How to Take
Numira tablets should be taken orally, with or without food. They should be swallowed whole with water. Do not crush or chew the tablet.
Mechanism of Action
Memantine is a non-competitive N-methyl-D-aspartate (NMDA) receptor antagonist. It blocks the effects of pathologically elevated tonic levels of glutamate, which may lead to neuronal dysfunction, without interfering with normal physiological neurotransmission.
Pharmacokinetics
Onset
Clinical effects are typically observed over several weeks of treatment.
Excretion
Primarily renal excretion of unchanged drug and metabolites. Renal clearance is influenced by urinary pH.
Half life
Approximately 60-80 hours.
Absorption
Well absorbed from the gastrointestinal tract. Absolute bioavailability is approximately 100%. Peak plasma concentration (Tmax) is reached in 3-7 hours.
Metabolism
Minimal hepatic metabolism, with some N-demethylation and conjugation reactions. Major metabolites are not pharmacologically active.
Side Effects
Contraindications
- •Hypersensitivity to memantine or any of the excipients.
Drug Interactions
Hydrochlorothiazide
May decrease memantine levels.
L-dopa, Dopaminergic Agonists
May enhance the effects of these drugs.
Amantadine, Ketamine, Dextromethorphan
Concomitant use with other NMDA-antagonists should be avoided due to the risk of enhanced pharmacodynamic effects and adverse reactions.
Anticholinergics, Barbiturates, Neuroleptics
Memantine may modify the effects of these drugs.
Cimetidine, Ranitidine, Procainamide, Quinidine, Nicotine
May increase memantine plasma levels due to competition for renal tubular secretion.
Storage
Store below 30°C in a dry place, away from direct light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include dizziness, somnolence, vomiting, diarrhea, confusion, agitation, hallucinations, gait disturbance, and rarely seizures. In case of overdose, discontinue the drug and provide symptomatic and supportive treatment. Gastric lavage and activated charcoal may be considered.
Pregnancy & Lactation
Pregnancy Category B. Studies in animals have not shown direct or indirect harmful effects with respect to pregnancy, embryonal/foetal development. However, human data are limited. Numira should be used during pregnancy only if clearly needed. It is not known whether memantine is excreted in human breast milk, but it is excreted in animal milk. Breastfeeding is not recommended during treatment.
