Overview of the medicine
Numira 5 mg Tablet is used to reduce the amount of acid produced in the stomach. It treats conditions like gastro-oesophageal reflux disease (GORD), heartburn, and stomach ulcers by blocking the proton pump responsible for acid secretion.
Uses & Indications
Dosage
Adults
Typically 20-40 mg once daily for most adult indications. 5 mg strength is not a standard adult therapeutic dose but might be considered for specific low-dose maintenance or pediatric use. Always follow physician's advice.
Elderly
No specific dose adjustment is generally required, but caution should be exercised in elderly patients with hepatic impairment.
Pediatric
For children 1-11 years weighing <20 kg: 5 mg once daily for GORD for up to 8 weeks. For children 1-11 years weighing ≥20 kg: 10 mg once daily for GORD for up to 8 weeks.
Renal_impairment
No dose adjustment is usually needed for patients with renal impairment.
Hepatic_impairment
For patients with severe hepatic impairment, a maximum dose of 20 mg daily is recommended. (Note: 5mg is generally safe but dose might be lower in severe cases)
How to Take
Numira 5 mg Tablet should be taken orally, usually once daily, at least one hour before a meal. Swallow the tablet whole with water; do not crush, chew, or break it.
Mechanism of Action
Esomeprazole, a proton pump inhibitor, irreversibly binds to and inhibits the H+/K+-ATPase (proton pump) in gastric parietal cells, thereby blocking the final step of acid secretion.

Onset
Acid suppression begins within 1 hour after oral administration.
Excretion
Excreted primarily in urine (approximately 80%) and feces (approximately 20%).
Half life
Approximately 1-1.5 hours.
Absorption
Rapidly absorbed after oral administration. Bioavailability is about 64% after a single dose and increases to about 90% after repeated dosing.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 system, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to esomeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with nelfinavir (an antiretroviral).
Drug Interactions
Warfarin
Increased INR and prothrombin time, requiring monitoring.
Clopidogrel
May reduce the antiplatelet effect of clopidogrel (due to CYP2C19 inhibition).
Nelfinavir/Atazanavir
Decreased plasma levels of these antiretrovirals; concomitant use with nelfinavir is contraindicated.
Diazepam, Phenytoin, Citalopram, Imipramine
Increased plasma levels due to CYP2C19 inhibition, requiring dose adjustment.
Drugs requiring acidic gastric pH (e.g., Ketoconazole, Iron salts, Erlotinib)
Decreased absorption of these drugs.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include drowsiness, confusion, blurred vision, tachycardia, nausea, sweating, flushing, headache, dry mouth, and other gastrointestinal disturbances. Management is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category B. Use during pregnancy only if clearly needed. Esomeprazole is excreted in human breast milk; caution should be exercised when administered to a nursing woman.
