Description
Overview of the medicine
Aripiprazole, marketed as Numira, is an atypical antipsychotic medication used to treat conditions such as schizophrenia, bipolar I disorder, and as an adjunctive treatment for major depressive disorder. It helps to restore the balance of certain natural substances in the brain.
Uses & Indications
Dosage
Adults
Schizophrenia: Initial 10-15 mg once daily, target dose 10-30 mg/day. Bipolar Mania: Initial 15 mg once daily, target dose 15-30 mg/day. Major Depressive Disorder (adjunctive): Initial 2-5 mg once daily, target dose 2-15 mg/day.
Elderly
Lower initial doses (e.g., 5-10 mg/day) and slower titration may be required due to increased sensitivity and potential for adverse effects.
Renal_impairment
No dosage adjustment is needed for patients with mild to moderate renal impairment. Use with caution in severe renal impairment.
How to Take
Take orally once daily, with or without food. Swallow the tablet whole with water.
Mechanism of Action
Aripiprazole acts as a partial agonist at dopamine D2 and serotonin 5-HT1A receptors, and as an antagonist at serotonin 5-HT2A receptors. This unique mechanism is believed to contribute to its efficacy in managing psychotic symptoms and mood disorders.
Pharmacokinetics
Onset
Initial effects may be observed within 1-2 weeks, but full therapeutic effect can take several weeks.
Excretion
Excreted through feces (approximately 55%) and urine (approximately 25%).
Half life
Approximately 75 hours for aripiprazole and 94 hours for its active metabolite, dehydro-aripiprazole.
Absorption
Well absorbed orally; peak plasma concentrations achieved in 3-5 hours.
Metabolism
Extensively metabolized in the liver, primarily by CYP2D6 and CYP3A4 enzymes.
Side Effects
Contraindications
- •Known hypersensitivity to aripiprazole or any component of the formulation.
- •Severe cardiac insufficiency.
Drug Interactions
CNS depressants
Additive effects with alcohol and other CNS depressants (e.g., benzodiazepines) leading to increased sedation.
CYP3A4/2D6 inducers
May decrease aripiprazole plasma concentrations (e.g., carbamazepine, rifampin). Dosage adjustment of aripiprazole may be necessary.
CYP3A4/2D6 inhibitors
May increase aripiprazole plasma concentrations (e.g., ketoconazole, quinidine). Dosage adjustment of aripiprazole may be necessary.
Storage
Store at room temperature (below 30°C) in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include somnolence, vomiting, tremor, and tachycardia. Management is primarily supportive, including maintaining an open airway, monitoring cardiac status, and providing symptomatic treatment. There is no specific antidote.
Pregnancy & Lactation
Use in pregnancy only if the potential benefit justifies the potential risk to the fetus. Aripiprazole is excreted in breast milk; a decision should be made whether to discontinue nursing or the drug, considering the importance of the drug to the mother.
