Description
Overview of the medicine
Cefixime is an oral third-generation cephalosporin antibiotic used to treat various bacterial infections susceptible to Cefixime.
Uses & Indications
Dosage
Adults
200 mg-400 mg once daily or 200 mg twice daily. Duration depends on the infection, typically 7-14 days.
Elderly
Dosage adjustments usually not required unless severe renal impairment is present. Monitor renal function.
Renal_impairment
For creatinine clearance 21-60 mL/min, 200 mg once daily. For creatinine clearance < 20 mL/min, 100 mg once daily.
How to Take
Odacef-200 mg capsule may be taken with or without food. To minimize gastric upset, it can be taken with food.
Mechanism of Action
Cefixime exerts its bactericidal effect by inhibiting bacterial cell wall synthesis. It binds to penicillin-binding proteins (PBPs), disrupting the cross-linking of peptidoglycans, which are essential for cell wall integrity.
Pharmacokinetics
Onset
Peak plasma concentrations reached 2-6 hours after administration.
Excretion
Excreted predominantly in the urine (50-60% unchanged) and bile.
Half life
Approximately 3-4 hours.
Absorption
Approximately 40-50% absorbed orally, with absorption enhanced by food.
Metabolism
Minimal hepatic metabolism; primarily eliminated as unchanged drug.
Side Effects
Contraindications
- Hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions to penicillin or other beta-lactam antibiotics.
Drug Interactions
Nifedipine
May increase cefixime absorption.
Probenecid
May increase cefixime plasma levels by decreasing its renal excretion.
Carbamazepine
Concomitant administration may result in increased carbamazepine levels.
Warfarin and other anticoagulants
May increase prothrombin time (PT) and INR, increasing the risk of bleeding. Close monitoring of coagulation parameters is recommended.
Storage
Store in a cool, dry place below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
In case of overdose, gastric lavage may be indicated. Cefixime is not removed in significant quantities by hemodialysis or peritoneal dialysis. Treatment should be symptomatic and supportive.
Pregnancy & Lactation
Pregnancy category B. Studies in animals have not shown teratogenic effects. Use during pregnancy only if clearly needed. Cefixime is excreted in breast milk; use with caution in nursing mothers.

Cefixime has undergone extensive clinical trials demonstrating its efficacy and safety for various bacterial infections. Ongoing research focuses on its use in emerging resistant strains and new formulations.
Lab Monitoring
- Renal function tests (e.g., creatinine clearance) should be monitored in patients with pre-existing renal impairment or during prolonged high-dose therapy.
- Blood counts may be monitored during prolonged therapy.
Doctor Notes
- Culture and susceptibility testing are recommended prior to initiating therapy, if feasible, to guide appropriate treatment.
- Monitor for signs of hypersensitivity reactions, including skin rash, urticaria, and anaphylaxis, especially in patients with a history of penicillin allergy.
- Advise patients to complete the full prescribed course to prevent bacterial resistance.
Patient Guidelines
- Take the full course of medication as prescribed, even if symptoms improve, to prevent resistance.
- Do not share this medication with others.
- Report any severe or persistent side effects to your doctor immediately.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for a missed one.
