Odacef 400 mg Capsule contains Cefixime, a third-generation cephalosporin antibiotic. It is used to treat a wide range of bacterial infections in various parts of the body.
Uses & Indications
Dosage
Adults
The usual adult dose is 400 mg once daily or 200 mg twice daily. Duration of treatment depends on the type and severity of infection, typically 7 to 14 days.
Elderly
Same as adults, but dose adjustment may be necessary in patients with severe renal impairment.
Renal_impairment
For creatinine clearance 21-60 mL/min, 300 mg daily. For creatinine clearance <20 mL/min, 200 mg daily. Hemodialysis and peritoneal dialysis patients may require dose adjustment.
How to Take
Odacef 400 mg Capsule is for oral administration. It can be taken with or without food. To ensure complete absorption and effectiveness, it should be swallowed whole with water.
Mechanism of Action
Cefixime works by inhibiting bacterial cell wall synthesis, leading to bacterial cell death (bactericidal action). It binds to penicillin-binding proteins (PBPs) located inside the bacterial cell wall, which interferes with the final transpeptidation step of peptidoglycan synthesis.
Pharmacokinetics
Onset
Peak plasma concentrations are usually achieved within 2-6 hours after oral administration.
Excretion
Approximately 50% of the absorbed dose is excreted unchanged in the urine over 24 hours, and about 10% is excreted in bile.
Half life
Plasma elimination half-life is typically 3-4 hours in healthy adults.
Absorption
Approximately 40-50% orally absorbed. Food slightly prolongs Tmax but does not significantly alter Cmax or AUC.
Metabolism
Minimal hepatic metabolism; Cefixime is primarily excreted as unchanged drug.
Side Effects
Contraindications
- Known hypersensitivity to Cefixime or any other cephalosporin antibiotics.
- History of severe hypersensitivity reactions (e.g., anaphylaxis) to penicillin or other beta-lactam antibiotics.
Drug Interactions
Carbamazepine
Concomitant use may increase carbamazepine plasma concentrations. Monitor carbamazepine levels.
Warfarin and other anticoagulants
May increase prothrombin time/INR, leading to increased bleeding risk. Monitor INR closely.
Aminoglycosides and Loop Diuretics
May increase risk of nephrotoxicity, especially in patients with impaired renal function.

Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.

In case of overdose, gastric lavage may be indicated. There is no specific antidote. Treatment should be symptomatic and supportive. Hemodialysis or peritoneal dialysis does not effectively remove Cefixime from the body.

Pregnancy Category B. Studies in animals have not shown harm to the fetus, but there are no adequate and well-controlled studies in pregnant women. Use only if clearly needed. Cefixime is excreted in small amounts in breast milk; caution should be exercised when administered to a nursing mother.
