Omastin 150 mg Capsule is an extended-release formulation of Omeprazole, a proton pump inhibitor (PPI) used to reduce stomach acid production. It is typically prescribed for severe acid-related conditions, such as Zollinger-Ellison Syndrome or refractory gastroesophageal reflux disease (GERD), where higher doses are required.
Uses & Indications
Dosage
Adults
For Zollinger-Ellison Syndrome, the initial recommended dose is 60 mg once daily. Doses can be adjusted based on individual response, with some patients requiring up to 120-150 mg once daily or divided doses. For severe erosive esophagitis/refractory GERD, doses higher than 40 mg daily may be considered by a physician, with 150 mg possibly used for extreme cases.
Elderly
No specific dosage adjustment based on age alone is typically required; however, caution is advised due to potential comorbidities and polypharmacy. Monitor for adverse effects.
Renal_impairment
No dosage adjustment is necessary for patients with impaired renal function.
How to Take
Take the capsule whole with a glass of water. Do not chew, crush, or open the capsule, as this may affect the extended-release properties. It is best taken before a meal, preferably in the morning, to ensure optimal acid suppression throughout the day.
Mechanism of Action
Omeprazole is a proton pump inhibitor that irreversibly binds to and inhibits the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. This action effectively blocks the final step in gastric acid secretion, significantly reducing both basal and stimulated acid production.

Acid suppression begins within 1-2 hours after administration. Full therapeutic effect typically within 2-4 days.
Excretion
Approximately 77% of the dose is excreted in the urine, and the remainder is excreted in the feces via biliary secretion.
Half life
Plasma elimination half-life is short (0.5 to 1 hour), but the duration of acid suppression is much longer (up to 72 hours) due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed from the gastrointestinal tract, but the extended-release formulation delays and prolongs absorption, leading to sustained plasma levels and therapeutic effect. Bioavailability is approximately 30-65%.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 enzyme system, specifically CYP2C19 and CYP3A4, into inactive metabolites.
Side Effects
Contraindications
- •Hypersensitivity to Omeprazole, substituted benzimidazoles, or any component of the formulation.
- •Concomitant use with Nelfinavir (an antiretroviral drug) due to significant reduction in Nelfinavir plasma concentrations.
Drug Interactions
Digoxin
Increased Digoxin levels due to increased gastric pH.
Warfarin

Methotrexate

Overdose
Limited information on overdose is available. Symptoms may include nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive. Omeprazole is not dialyzable.
