Omastin 50 mg capsule contains Orlistat, a medication used for weight loss by preventing the absorption of fats from the diet. It is typically used in conjunction with a reduced-calorie diet and exercise.
Uses & Indications
Dosage
Adults
One 50 mg capsule three times daily with each main meal containing fat. Can be taken during or up to one hour after the meal. If a meal is missed or contains no fat, the dose can be omitted.
Elderly
No specific dose adjustment required for elderly patients.
Renal_impairment
No specific dose adjustment required, but use with caution due to limited data.
How to Take
Take one capsule orally with water during or up to one hour after each main meal that contains fat. If a meal is missed or contains no fat, omit the dose for that meal.
Mechanism of Action
Orlistat inhibits gastrointestinal lipases, enzymes that break down dietary fat (triglycerides) in the intestine. By inhibiting these enzymes, Orlistat prevents the hydrolysis of triglycerides into absorbable free fatty acids and monoglycerides. Undigested fat is then excreted in the feces, leading to a reduction in calorie intake.

Onset
Within 24-48 hours of first dose (fat excretion begins)
Excretion
Primarily via feces (approximately 97% of the dose, 83% as unchanged drug). Renal excretion accounts for a minor portion (less than 2%).
Half life
Approximately 1-2 hours (for the small amount absorbed systemically)
Absorption
Minimal systemic absorption (less than 1%) after oral administration. Most of the drug remains in the gastrointestinal tract.
Metabolism
Minimal systemic metabolism. Primarily metabolized within the gastrointestinal wall to inactive metabolites (M1 and M3).
Side Effects
Contraindications
- •Chronic malabsorption syndrome
- •Cholestasis
- •Pregnancy and lactation
- •Hypersensitivity to Orlistat or any component of the formulation
- •Concomitant use with cyclosporine
- •Nephrolithiasis (history of kidney stones) in obese individuals
Drug Interactions
Amiodarone
Reduced absorption of amiodarone has been reported.
Cyclosporine
Orlistat may decrease the absorption of cyclosporine. Administer cyclosporine at least 3 hours after Orlistat.
Levothyroxine
May decrease absorption of levothyroxine. Administer levothyroxine at least 4 hours apart from Orlistat.
Warfarin and other oral anticoagulants
Vitamin K absorption may be reduced, requiring closer monitoring of INR.
Fat-soluble vitamins (A, D, E, K) and beta-carotene
Absorption may be reduced. Administer a multivitamin supplement containing these vitamins at least 2 hours before or after Orlistat, or at bedtime.
Storage
Store in a cool, dry place, below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Single doses of 800 mg and multiple doses of 400 mg three times daily for 15 days in normal weight and obese subjects have shown no significant adverse findings. However, if overdose occurs, it is recommended to observe the patient for 24 hours. Systemic effects related to lipase inhibition are rapidly reversible.
Pregnancy & Lactation
Orlistat is contraindicated during pregnancy and lactation due to potential for fetal harm and unknown effects on breastfed infants. Weight loss during pregnancy is not recommended.
