Description
Overview of the medicine
Omegut 40 mg Capsule contains Omeprazole, a proton pump inhibitor (PPI) used to reduce stomach acid production. It is prescribed for various acid-related conditions such as gastroesophageal reflux disease (GERD), stomach ulcers, and conditions caused by excess stomach acid.
Uses & Indications
Dosage
Adults
The usual adult dosage for conditions like severe GERD, erosive esophagitis, or Zollinger-Ellison Syndrome is 40 mg once daily. Dosage and duration depend on the condition and response to treatment.
Elderly
No specific dose adjustment is generally required for elderly patients, but caution is advised, and monitoring for adverse effects is recommended.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Omegut 40 mg Capsule should be taken orally, usually once daily, preferably in the morning before a meal. Swallow the capsule whole with water; do not chew or crush it.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase proton pump located in the secretory canaliculi of gastric parietal cells. This action effectively blocks the final step in gastric acid secretion, regardless of the stimulus.
Pharmacokinetics
Onset
Acid secretion inhibition begins within 1 hour, with maximum effect within 2 hours. Duration of effect is up to 72 hours despite a short plasma half-life.
Excretion
Approximately 80% of metabolites are excreted in the urine and the remainder in the feces.
Half life
Plasma elimination half-life is approximately 0.5 to 1 hour.
Absorption
Rapidly absorbed after oral administration. Bioavailability is approximately 30-40% due to first-pass metabolism but increases with repeated dosing.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 system (CYP2C19 and CYP3A4).
Side Effects
Contraindications
- Hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with Nelfinavir.
Drug Interactions
Digoxin
Omeprazole may increase digoxin absorption, leading to higher digoxin levels.
Warfarin
Potential for increased INR and bleeding risk; close monitoring is advised.
Tacrolimus
May increase tacrolimus plasma concentrations.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19, potentially increasing cardiovascular event risk.
Methotrexate
Concomitant use may increase methotrexate levels, especially with high doses, requiring monitoring.
Storage
Store below 30°C. Protect from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and self-limiting, including nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy Category C. Omeprazole should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus. It is excreted in human milk, so caution should be exercised when administered to a nursing woman.

- If used long-term (e.g., >1 year), monitor serum magnesium levels periodically.
- Consider monitoring vitamin B12 levels in patients on prolonged therapy.
- Liver function tests may be considered in patients with hepatic impairment.
Doctor Notes
- Advise patients to take Omegut 40 mg capsule before food for optimal efficacy.
- Regularly assess the need for continued PPI therapy, especially for long-term users, to mitigate potential risks.
- Be aware of potential drug interactions, particularly with clopidogrel and methotrexate.
Patient Guidelines
- Take the capsule exactly as prescribed by your doctor.
- Do not chew, crush, or open the capsule; swallow it whole.
- If you are on long-term therapy, do not stop taking the medicine suddenly without consulting your doctor.
- Inform your doctor about all other medications you are taking.
