Description
Overview of the medicine
Omeprol 40 mg Capsule contains Omeprazole, a proton pump inhibitor used to reduce stomach acid production. It is prescribed for conditions like gastroesophageal reflux disease (GERD), stomach ulcers, and Zollinger-Ellison syndrome.
Uses & Indications
Dosage
Adults
For GERD: 20-40 mg once daily for 4-8 weeks. For Duodenal Ulcer: 20 mg once daily for 2-4 weeks. For Gastric Ulcer: 20 mg once daily for 4-8 weeks. For Zollinger-Ellison Syndrome: Initial dose 60 mg once daily, adjusted based on patient response.
Elderly
No specific dose adjustment is required, but caution is advised in elderly patients with hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take the capsule whole with a glass of water, preferably in the morning before food. Do not chew or crush the capsule.
Mechanism of Action
Omeprazole is a proton pump inhibitor (PPI) that irreversibly binds to and inhibits the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. This inhibition prevents the final step of acid secretion, thereby reducing gastric acidity.
Pharmacokinetics
Onset
Within 1 hour
Excretion
Mainly through urine (approx. 77%) and feces (approx. 23%) as metabolites.
Half life
0.5-1 hour (plasma half-life), but duration of acid inhibition is longer due to irreversible binding to proton pump.
Absorption
Rapidly but variably absorbed from the gastrointestinal tract; bioavailability is about 30-40% but increases with repeated dosing.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4 enzymes.
Side Effects
Contraindications
- Hypersensitivity to Omeprazole or any component of the formulation
- Concomitant use with Nelfinavir
Drug Interactions
Digoxin
Increased digoxin levels.
Warfarin
Increased INR and bleeding risk.
Nelfinavir
Significantly reduced plasma concentration of nelfinavir.
Clopidogrel
Reduced antiplatelet effect due to CYP2C19 inhibition by omeprazole.
Methotrexate
Increased methotrexate levels.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose include nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. Treatment is largely symptomatic and supportive. No specific antidote is known.
Pregnancy & Lactation
Pregnancy Category C. Use only if potential benefit justifies potential risk to the fetus. Small amounts are excreted in breast milk; caution should be exercised when administered to a nursing mother.

YesClinical Trials
Numerous clinical trials have established the efficacy and safety of omeprazole for various acid-related disorders. Ongoing research focuses on long-term effects and comparisons with other PPIs.
Lab Monitoring
- Liver function tests (in patients with hepatic impairment)
- Magnesium levels (with long-term use)
- Chromogranin A (CgA) levels (can interfere with neuroendocrine tumor diagnosis)
Doctor Notes
- Counsel patients on potential long-term risks with PPIs.
- Consider lowest effective dose and shortest duration of therapy.
- Assess for drug interactions, especially with antiplatelets and antivirals.
Patient Guidelines
- Take as directed by your doctor.
- Do not stop taking the medicine suddenly without consulting your doctor.
- Inform your doctor about all other medications you are taking.
- Report any severe or persistent side effects.
Missed Dose Advice
