Overview of the medicine
Omeprazole is a proton pump inhibitor (PPI) that reduces the amount of acid produced in the stomach. It is used to treat conditions such as heartburn, acid reflux, and ulcers.
Uses & Indications
Dosage
Adults
Typical adult dosage is 20 mg once daily, taken before a meal. For severe conditions like Zollinger-Ellison Syndrome, higher doses (e.g., 60 mg once daily or divided) may be prescribed.
Elderly
No specific dose adjustment is generally required for elderly patients, but caution is advised in those with severe hepatic impairment.
Renal_impairment
No dose adjustment is necessary for patients with impaired renal function.
How to Take
Take one capsule orally, usually in the morning before breakfast, with a glass of water. Do not chew or crush the capsule; swallow it whole.
Mechanism of Action
Omeprazole works by irreversibly binding to and inhibiting the H+/K+-ATPase (proton pump) in the gastric parietal cells. This action prevents the final step of acid secretion, leading to a significant reduction in stomach acid.
Pharmacokinetics
Onset
Acid inhibitory effect starts within 1 hour, with maximum effect within 2 hours. Full acid suppression achieved after 4 days of treatment.
Excretion
Approximately 80% of the metabolites are excreted in the urine and about 20% in the feces.
Half life
Plasma elimination half-life is typically 0.5 to 1 hour, but the duration of acid suppression is much longer (up to 72 hours) due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed from the gastrointestinal tract, with peak plasma concentrations occurring within 0.5 to 3.5 hours. Bioavailability is approximately 30-40% for a single dose but increases with repeated administration.
Metabolism
Extensively metabolized in the liver, primarily by the cytochrome P450 enzymes CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Known hypersensitivity to omeprazole, substituted benzimidazoles, or any component of the formulation.
- Concomitant use with nelfinavir.

Digoxin
Increased digoxin levels; monitor closely.
Warfarin
Increased INR/prothrombin time; monitor closely.
Tacrolimus
Increased tacrolimus levels; monitor closely.
Clopidogrel

Methotrexate

Pregnancy Category C. Use during pregnancy only if the potential benefit justifies the potential risk to the fetus. Omeprazole is excreted in human breast milk; a decision should be made whether to discontinue nursing or the drug, taking into account the importance of the drug to the mother.
