Overview of the medicine
Omex 40 mg Injection contains omeprazole, a proton pump inhibitor used to reduce stomach acid production. It is indicated for conditions like ulcers, GERD, and Zollinger-Ellison syndrome, especially when oral therapy is not feasible.
Uses & Indications
Dosage
Adults
Typically 40 mg once daily for most indications. For severe Zollinger-Ellison Syndrome, higher doses may be required, up to 60 mg every 8 hours.
Elderly
No specific dose adjustment is generally required, but caution is advised with severe hepatic impairment.
Renal_impairment
No dose adjustment needed for renal impairment.
How to Take
For intravenous infusion only after reconstitution. Administer slowly over 20-30 minutes. Do not administer as a bolus injection.
Mechanism of Action
Omeprazole is a proton pump inhibitor that irreversibly blocks the H+/K+-ATPase enzyme system (the proton pump) in the gastric parietal cells. This reduces gastric acid secretion by inhibiting the final step in acid production.
Pharmacokinetics
Onset
Within 30-60 minutes after IV administration.
Excretion
Excreted mainly in the urine (approx. 77%) and feces (approx. 18%) as metabolites.
Half life
Approximately 0.5 to 1 hour (terminal half-life), but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after intravenous administration, 100% bioavailability. Oral bioavailability is ~30-40% due to first-pass metabolism.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- Hypersensitivity to omeprazole or any component of the formulation
- Concomitant use with nelfinavir
Drug Interactions
Warfarin
Increased INR/prothrombin time; monitor closely.
Clopidogrel
Omeprazole may reduce the antiplatelet effect of clopidogrel by inhibiting CYP2C19.
Methotrexate
Increased methotrexate levels; consider temporary discontinuation of PPI or lower methotrexate dose.
Nelfinavir/Atazanavir
Reduced plasma levels of antiretrovirals; co-administration is generally contraindicated.
