Description
Overview of the medicine
Omeprazole is a proton pump inhibitor (PPI) that decreases the amount of acid produced in the stomach. It is used to treat conditions such as heartburn, stomach ulcers, and gastroesophageal reflux disease (GERD).
Uses & Indications
Dosage
Adults
Dosage varies by indication. For GERD: 20-40 mg once daily for 4-8 weeks. For Duodenal Ulcer: 20 mg once daily for 2-4 weeks. For Gastric Ulcer: 20 mg once daily for 4-8 weeks. For H. pylori eradication: 20 mg twice daily in combination with antibiotics. For Zollinger-Ellison Syndrome: Initial 60 mg once daily, adjusted based on response.
Elderly
No specific dose adjustment is necessary for elderly patients unless they have severe hepatic impairment.
Renal_impairment
No dosage adjustment is required for patients with impaired renal function.
How to Take
Take the capsule whole, usually once daily in the morning before a meal. Do not chew or crush the capsule.
Mechanism of Action
Omeprazole is a substituted benzimidazole that suppresses gastric acid secretion by specific inhibition of the H+/K+-ATPase (proton pump) at the secretory surface of the parietal cells. This effect is dose-related and leads to inhibition of both basal and stimulated acid secretion, regardless of the stimulus.
Pharmacokinetics
Onset
Within 1-2 hours for acid suppression.
Excretion
Approximately 77% of the dose is excreted in the urine as metabolites, and the remainder is excreted in the feces.
Half life
Plasma half-life is approximately 0.5-1 hour, but the duration of acid suppression is much longer due to irreversible binding to the proton pump.
Absorption
Rapidly absorbed after oral administration, peak plasma concentrations occur within 0.5-3.5 hours. Bioavailability increases with repeated dosing due to reduced first-pass metabolism.
Metabolism
Extensively metabolized in the liver by the cytochrome P450 (CYP) system, primarily by CYP2C19 and CYP3A4, to inactive metabolites.
Side Effects
Contraindications
- Hypersensitivity to omeprazole or any component of the formulation.
- Concomitant use with nelfinavir (an antiretroviral drug).
Drug Interactions
Digoxin
Increased digoxin bioavailability.
Diazepam
Delayed elimination of diazepam.
Warfarin
Increased INR and bleeding risk.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased methotrexate levels, potential toxicity.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and include nausea, vomiting, dizziness, abdominal pain, diarrhea, and headache. There is no specific antidote; treatment should be symptomatic and supportive.

YesClinical Trials
Omeprazole has undergone extensive clinical trials demonstrating its efficacy and safety for various acid-related disorders since its introduction. Ongoing research continues to explore its long-term effects and potential new applications.
Lab Monitoring
- Routine lab monitoring is not typically required. However, for long-term use, magnesium levels, vitamin B12 levels, and bone density may be monitored.
Doctor Notes
- Prescribe the lowest effective dose for the shortest duration necessary.
- Consider magnesium and vitamin B12 supplementation for long-term PPI users.
- Monitor for signs of C. difficile infection in patients presenting with persistent diarrhea.
- Counsel patients on lifestyle modifications to complement therapy.
Patient Guidelines
- Take exactly as prescribed by your doctor.
- Swallow the capsule whole; do not chew, crush, or open it.
- Take it before a meal, preferably in the morning.
- Report any severe or persistent side effects to your doctor.
- Do not stop taking the medicine abruptly without consulting your doctor.
Missed Dose Advice
If you miss a dose, take it as soon as you remember. If it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.
