Overview of the medicine
OPmax (Omeprazole Extended-Release) is a proton pump inhibitor used to treat conditions caused by too much stomach acid. It helps reduce acid production in the stomach, providing relief from symptoms like heartburn, indigestion, and healing ulcers.
Uses & Indications
Dosage
Adults
For GERD: 20 mg once daily for 4-8 weeks. For ulcers: 20 mg once daily for 2-4 weeks. Take before breakfast.
Elderly
No specific dose adjustment is needed, but caution advised due to potential for reduced liver function.
Renal_impairment
No dose adjustment is necessary for patients with renal impairment.
How to Take
Take one capsule orally, swallowed whole with water, at least 30 minutes before breakfast. Do not crush or chew the capsule.
Mechanism of Action
Omeprazole inhibits the H+/K+-ATPase enzyme system (the 'proton pump') in the gastric parietal cells, thereby blocking the final step of acid production.
Pharmacokinetics
Onset
Onset of acid suppression within 1 hour, maximum effect within 2 hours.
Excretion
Mainly excreted in urine (approximately 80%) and feces (approximately 20%).
Half life
Plasma half-life is approximately 0.5-1 hour, but duration of acid suppression is much longer.
Absorption
Rapidly absorbed after oral administration. Bioavailability increases with repeated dosing.
Metabolism
Extensively metabolized in the liver, primarily by CYP2C19 and CYP3A4.
Side Effects
Contraindications
- •Hypersensitivity to omeprazole or any component of the formulation.
- •Concomitant use with nelfinavir.
Drug Interactions
Diazepam
Prolonged elimination of diazepam.
Warfarin
Increased INR and prothrombin time.
Clopidogrel
Reduced antiplatelet effect of clopidogrel.
Methotrexate
Increased plasma concentration of methotrexate.
Storage
Store below 30°C in a dry place, away from light and moisture. Keep out of reach of children.
Overdose
Symptoms of overdose are generally mild and include nausea, vomiting, dizziness, abdominal pain, and headache. Treatment is symptomatic and supportive.
Pregnancy & Lactation
Pregnancy category C. Use during pregnancy only if clearly needed. Omeprazole is excreted in human milk; caution should be exercised when administered to a nursing woman.
